改良双切口锁定钢板内固定治疗胫骨远端骨折

The locking plate internal fixation for treatment of distal tibial fracture with modified double incisions

目的探讨改良双切口锁定钢板内固定治疗胫骨远端骨折的疗效。方法将108例胫骨远端骨折患者按照术式不同分为观察组(采用改良双切口锁定钢板内固定治疗,51例)和对照组(采用传统手术治疗,57例)。记录两组术中出血量、手术时间、住院时间、骨折复位情况、骨折愈合情况。采用疼痛VAS评分、ODI评分、踝关节活动度评价临床疗效。结果患者均获得随访,时间8~28(13.52±3.41)个月。术中出血量、手术时间、住院时间两组比较差异均无统计学意义(P>0.05)。骨折复位情况、骨折愈合情况及末次随访时的疼痛VAS评分、ODI评分、踝关节活动度观察组明显优于对照组(P<0.05)。结论采用改良双切口锁定钢板内固定治疗胫骨远端骨折在恢复踝部功能、缓解疼痛及促进骨折愈合等方面疗效满意,还可有效规避手术创伤对康复的影响,降低不良反应事件的发生率。

Objective To investigate the effect of locking plate internal fixation for distal tibial fracture with modified double incisions.Methods A total of 108 patients with distal tibial fractures were divided into observation group(using modified double incision locking plate internal fixation,51 cases)and control group(using traditional surgical treatment,57 cases),according to different operation methods.The intraoperative bleeding volume,operation time,hospital stay,fracture reduction and fracture healing of the two groups were recorded.The pain VAS,ODI score and the ankle range of motion were used to evaluate the clinical efficacy.Results All patients were followed up for 8~28(13.52±3.41)months.There were no significant differences in intraoperative blood loss,operation time or hospital stay time between the two groups(P>0.05).The observation group were significantly better than the control group in the fracture reduction,fracture healing,pain VAS at the last follow-up,ODI score and the ankle range of motion(P<0.05).Conclusions In the treatment of distal tibial fracture,locking plate internal fixation via modified double incisions have the satisfactory efficacy,which can restore the ankle function,relieve the pain symptoms and promote of fracture healing,and which can effectively avoid the influence of the surgical trauma on rehabilitation,and reduce the incidence of adverse reaction events.
