左心室机械同步性与左束支阻滞的关系

The relationship between left ventricular mechanical synchronization and left bundle branch block

摘要 目的探究依据传统标准和新标准诊断的左束支阻滞,及其与超声心动图评价的心脏机械失同步指标的一致性。方法选取2018年6月至2019年6月于我院植入CRT-P/D装置的患者39例,依据不同标准分为"真性"LBBB组(真LBBB)和"假性"LBBB组(假LBBB),采用超声心动图技术评价两组患者心脏同步性指标。结果 M型超声、组织多普勒和实时三维超声检查结果均显示,"真性"LBBB组左心室收缩失同步显著大于"假性"LBBB组,差异有统计学意义。结论"真性"LBBB组左心室收缩失同步显著大于"假性"LBBB组。 Objective To explore the correlation between ECG left bundle-branch block and left ventricular mechanical dyssynchrony by echocardiography in patients with chronic heart failure. Methods A retrospective analysis was conducted on 39 patients with chronic heart failure admitted to our hospital from June 2018 to June2019. They were divided into “true”LBBB(t-LBBB) and “false”LBBB(f-LBBB) group according to QRS morphology. A comparison was made according to the two groups. Results Patients with “true”LBBB(n=21) had a greater left ventricular mechanical dyssynchrony compared with patients with “false”LBBB(n=18)(p<0.01).Conclusions Left ventricular mechanical dyssynchrony detected by echocardiography is greater in patients with“true”LBBB than in those with“false”LBBB.
