摘要
目的:观察青海地区薄型子宫内膜患者宫腔灌注粒细胞集落刺激因子(G-CSF)的治疗效果。方法:收集2018年4月—2020年4月我院收治的60例薄型子宫内膜患者,随机分为观察组(n=30)与对照组(n=30),对照组给予单纯雌孕激素治疗,观察组给予宫腔灌注G-CSF+雌孕激素治疗,比较两组患者治疗后的子宫内膜形态、子宫内膜厚度、血流动力学指标及变化情况。结果:两组患者治疗前子宫内膜厚度无统计学差异(P>0.05),但治疗后2天、排卵日、LH峰日时两组子宫内膜厚度存在明显差异,差异有统计学意义(P<0.05)。治疗前两组患者的血流动力学指标比较,差异无统计学意义(P>0.05);治疗后2天,观察组的阻力指数(RI)、脉动指数(PI)、收缩期峰值速度(PSV)的子宫血流动力学指标明显优于对照组,差异具有统计学意义(P<0.05)。观察组患者的治疗总有效率为86.67%,对照组患者治疗总有效率为70.00%,差异无统计学意义(P> 0.05)。结论:青海地区薄型子宫内膜患者应用雌孕激素联合宫腔灌注G-CSF治疗效果显著,能够改善患者的子宫内膜形态、厚度及血流动力学指标,值得临床应用推广。
Objective:This study is aimed to observe the therapeutic effect of intrauterine perfusion of granulocyte colony stimulating factor in patients with thin endometrium at Qinghai altitude.Methods:60 patients with thin endometrium treated in our hospital from April 2018 to April 2020 were randomly divided into observation group(n =30)and control group(n =30).The control group was treated with estrogen and progesterone alone,while the observation group was treated with intrauterine infusion of recombinant granulocyte colony stimulating factor + estrogen and progesterone to compare the endometrial morphology,endometrial thickness and Hemodynamic indexes and changes of these two groups.Results:There was no significant difference in endometrial thickness between the two groups before treatment,but there was significant difference in endometrial thickness between the two groups after 2 days of treatment,ovulation day and LH peak day(P< 0.05).There was no difference in hemodynamic index scores between the two patients before treatment(P >0.05).Two days after treatment,the uterine hemodynamic indexes of RI,PI and PSV in the observation group were significantly better than those in the control group(P < 0.05).The total effective rate was 86.67% in the observation group and 70.00% in the control group(P > 0.05).Conclusion:The application of estrogen and progesterone combined with intrauterine perfusion of granulocyte colony stimulating factor in patients with thin endometrium at Qinghai altitude is effective, which can improve endometrial morphology,endometrial thickness and Hemodynamic indexes,and it is worthy of clinical application and promotion.
