摘要
目的:探讨高原创伤失血性休克猪七氟醚摄取量变化,为高原创伤失血性休克患者临床使用七氟醚提供依据和指导。方法:选取10只雄性巴马香猪,采用随机数字量表法分为对照组和创伤合并35%失血量组(休克组)各5只。采用容量控制性失血方法建立创伤失血性休克模型,休克组创伤后10min内匀速放出35%全身血容量,对照组不创伤不放血。使用Narcotrend麻醉深度监测仪监测实验动物麻醉深度,调整七氟醚浓度使实验动物NT指数维持在40~45之间,观察并比较两组实验动物呼出(Fa)及吸入(Fi)七氟醚浓度变化,并计算Fa/Fi值。结果:与对照组比较,休克组40min及40min后平均动脉压(MAP)显著降低,心率(HR)显著升高,差异有统计学意义(P<0.05);与对照组比较,休克组实验动物Fa与Fi在实验开始30min后均升高,差异无统计学意义(P>0.05);休克组Fa/Fi在实验开始30min后升高,差异无统计学意义(P>0.05)。结论:高原创伤失血性休克猪与高原未创伤猪在吸入七氟醚浓度、呼出七氟醚浓度及七氟醚摄取量上无变化。
Objective:The purpose of this project is to study the changes of sevoflurane intake in pigs with traumatic hemorrhagic shock at high altitude,to provide basis and guidance for clinical use of sevoflurarne in patients with traumatic hemorrhagic shock at high altitule.Methods:Ten male Bama Xiang pigs were randomly divided into control group and trauma group which was combined with 35% blood loss(shock group),with 5 pigs in each group.The traumatic hemorrhagic shock model was established by volume controlled blood loss.35% of the whole body blood volume was released at a uniform rate within 10 minutes after trauma in the shock group and no blood was released in the control group.Narcotrend anesthesia depth monitor was used to monitor the anesthesia depth of experimental animals,adjusting the sevoflurane concentration to maintain the NT index of experimental animals between 40 and 45,observe the changes of exhaled(Fa)and inhaled(Fi)sevoflurane concentration of two groups of experimental animals,and calculate the Fa/Fi value.Results:Compared with the control graup,MAP and HR increased sighificantly in the shock group 40 min and 40 min after the beginning of the experment,the difference was statistically significant(P < 0.05). Compared with the control group,FA and fi in the shock group increased 30 min after the beginning of the experiment,without the statistically significant difference(P > 0.05);Fa/Fi increased 30 min after the beginning of the experiment,and the difference was not statistically significant(P > 0.05).Conclusion:There was no significant difference in inhaled sevoflurane concentration,exhaled sevoflurane concentration and sevoflurane intake between high altitude traumatic hemorrhagic shock pigs and high altitude non traumatic pigs.
