肺泡表面活性物质联合无创通气治疗新生儿呼吸窘迫综合征的疗效 被引量:2

摘要 目的分析肺泡表面活性物质联合无创通气治疗新生儿呼吸窘迫综合征的效果。方法选取福鼎市医院在2017年3月—2019年3月收治的100例新生儿呼吸窘迫综合征患儿,按照患儿入院先后顺序分为单一组和联合组,每组50例。单一组患儿行无创通气治疗,联合组患儿行肺泡表面活性物质联合无创通气治疗,比较2组患儿治疗后总有效率、血气分析〔pH值、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))〕、机械通气时间、用氧时间、住院时间、心率、呼吸频率、吸入氧浓度(FiO_(2))以及不良反应(气胸、坏死性小肠结肠炎、支气管肺发育不良)的发生情况。结果联合组患儿治疗后总治疗有效率高于单一组患儿(P<0.05);联合组患儿治疗后pH值、PaO_(2)水平高于单一组,PaCO_(2)水平低于单一组(P<0.05);联合组患儿治疗后机械通气时间、用氧时间与住院时间低于单一组(P<0.05);联合组患儿治疗后心率、呼吸频率、FiO_(2)低于单一组(P<0.05);2组患儿不良反应总发生率比较,差异无统计学意义(P>0.05)。结论在治疗新生儿呼吸窘迫综合征中应用肺泡表面活性物质联合无创通气的临床效果明显,患儿接受治疗后呼吸指标得到改善。 Objective To analyze the effect of alveolar surfactant combined with non-invasive ventilation in the treatment of neonatal respiratory distress syndrome.Methods A total of 100 cases of children with neonatal respiratory distress syndrome in the Fuding Hospital from March 2017 to March 2019 were selected.The children were divided into single group and combined group according to the order of admission,with 50 cases in each group.The single group underwent non-invasive ventilation,and the combined group underwent alveolar surfactant therapy combined with non-invasive ventilation.Total effective rate of treatment,blood gas analysis(pH value,PaO_(2),PaCO_(2)),mechanical ventilation time,oxygen time,hospital day,heart rate,respiratory frequency,FiO_(2) were compared between the two groups,and the incidence of adverse reactions were observed.Results After treatment,the total effective rate of treatment in the combined group was higher than that in the single group(P<0.05).The pH value and PaO_(2) of the children in the combined group were higher than those in the single group(P<0.05).The PaCO_(2) of the children in the combined group was lower than that in the single group(P<0.05).The time of mechanical ventilation,oxygen time and hospital day of the children in the combined group werelower than those in the single group(P<0.05).The heart rate,respiratory frequency and FiO_(2) of the children in the combined group werelower than those in the single group after treatment (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion The effect of applying alveolar surfactant combined with non-invasive ventilation in the treatment of neonatal respiratory distress syndrome disease is significant,and the respiratory index of the child is greatly improved after treatment.
