不同鼻腔冲洗方法治疗过敏性鼻炎的疗效

Efficacy of three nasal cavity irrigation methods in the treatment of allergic rhinitis

目的观察不同鼻腔冲洗方法治疗过敏性鼻炎(AR)的疗效。方法AR患者75例随机分为三组,每组25例。无冲洗组仅使用鼻用糖皮质激素及口服组胺H1受体拮抗剂;盐水冲洗组在此基础上给予40℃生理盐水冲洗鼻腔;药物冲洗组则在40℃生理盐水中加入鼻渊通窍颗粒冲洗鼻腔。4周为1个疗程。疗程结束后比较三组患者的鼻结膜炎症状总分(TSS)、鼻黏膜纤毛传输率(MTR)及鼻腔阻力(NR)情况。结果无冲洗组、盐水冲洗组和药物冲洗组治疗后TSS评分分别为(4.52±2.29)分、(3.36±1.78)分和(2.24±1.36)分,差异有统计学意义(P<0.05);MTR分别为(4.95±1.51)mm/min、(5.72±1.05)mm/min和(6.46±1.09)mm/min,差异有统计学意义(P<0.05);NR分别为(0.35±0.08)Pa·s^(-1)·cm^(-3)、(0.28±0.08)Pa·s^(-1)·cm^(-3)和(0.21±0.06)Pa·s^(-1)·cm^(-3),差异有统计学意义(P<0.01)。在冲洗过程中,仅有少数患者有轻微的鼻腔刺痛和耳闷,冲洗结束后不适症状可缓解。结论在常规方法治疗AR的基础上,以40℃生理盐水为冲洗液冲洗鼻腔可进一步改善患者的症状,提高患者MTR,降低NR,且在冲洗液中加入鼻渊通窍颗粒效果更好。

Objective To observe the efficacy of three nasal cavity irrigation methods in the treatment of allergic rhinitis(AR).Methods Seventy-five AR patients were randomly divided into three groups with 25 cases each.The patients in group A was treated with nasal glucocorticoid and oral histamine H1 receptor antagonist.On the basis of treatment in group A,nasal cavity irrigation was performed with normal saline at 40℃ in group B or with Biyuan Tongqiao granules in normal saline at 40℃ in group C.The treatment lasted for 4 weeks.After treatment, the efficacy was evaluated with the total symptom score of nasal mucosa inflammation(TSS),nasal mucosal ciliary transport rate(MTR) and nasal resistance(NR).Results The TSS scores for groups of A,B and C after treatment were(4.52±2.29) points,(3.36±1.78) points and(2.24±1.36) points(P<0.05),MTR were(4.95±1.51) mm/min,(5.72±1.05) mm/min and(6.46±1.09) mm/min, respectively(P<0.05),and NR were(0.35±0.08) Pa·s^(-1)·cm^(-3),(0.28±0.08) Pa·s^(-1)·cm^(-3)and(0.21±0.06) Pa·s^(-1)·cm^(-3)(P<0.05),respectively.During treatment, a few patients had slight nasal tingling and ear tightness, which could be relieved after nasal cavity irrigation.Conclusion On the basis of conventional treatment of AR,nasal cavity irrigation with normal saline solution at 40℃ can further improve patients’ symptoms, elevate MRT and reduce NR.Moreover, adding Biyuan Tongqiao granules into the rinse solution has a better effect.
