“人”字形平卧位和改良式低截石位配合腹腔镜结直肠癌根治术的效果比较

Comparison of the effect of"human"-shaped supine position and modified low lithotomy position combined with laparoscopic radical resection of colorectal cancer

目的分析结直肠癌患者行腹腔镜根治术治疗期间手术室护理过程中不同体位摆放配合的临床效果。方法抽选2018年3月—2019年7月于信宜市人民医院接受腹腔镜手术治疗的结直肠癌患者87例进行研究,根据入院病案编号随机分为2组,对照组采取改良式低截石位实施手术,观察组采取“人”字形平卧位实施手术。分别记录2组患者手术前体位摆放用时及手术期间血压波动情况,评估不同体位摆放舒适度及出现肢体酸麻感概率。结果观察组患者术前体位摆放用时(3.13±1.02)min,明显短于对照组的(6.35±1.05)min(P<0.05);观察组手术期间血压波动水平(3.04±1.05)mmHg,相比对照组的(10.74±1.06)mmHg较小(P<0.05)。观察组舒适度明显优于对照组,出现肢体酸麻感概率相比对照组较低(P<0.05)。结论结直肠癌患者接受腹腔镜根治术治疗时,采取人字形平卧体位,配合手术室护理干预能够有效减小术中血压波动,且对手术视野、操作等无干扰,有利于手术开展。

Objective To investigate the effect of"human"-shaped supine position and modified low lithotomy position combined with laparoscopic radical resection of colorectal cancer.Methods A selection of 87 patients with colorectal cancer who underwent laparoscopic surgery in our hospital from March 2018 to July 2019 were selected for the study.The patients were randomly divided into two groups according to their admission medical record numbers.The control group was implemented with a modified low lithotomy position.During the operation,the observation group performed the operation in a"human"-shaped prone position,and recorded the time spent in the position before the operation and the blood pressure fluctuations during the operation of the two groups of patients,and evaluated the comfort of the different positions of the patients in each group and the probability of limbs tingling.Results The observation group took(3.13±1.02)min before surgery,which was significantly shorter than the control group(6.35±1.05)min(P<0.05);the observation group’s blood pressure fluctuation level during surgery was(3.04±1.05)mmHg,compared with the control The group(10.74±1.06)mmHg was smaller(P<0.05).At the same time,patients in the observation group indicated that their body position comfort was significantly better than that of the control group,and the probability of limb soreness was lower than that of the control group(P<0.05).Conclusion When patients with colorectal cancer undergo laparoscopic radical resection,the herringbone prone position and the nursing intervention in the operating room can effectively reduce the patient's intraoperative blood pressure fluctuations,and there is no interference with the surgical field of vision and operation,which is beneficial to the operation.
