杞菊地黄丸联合玻璃酸钠滴眼液治疗白内障术后干眼的疗效观察

Clinical observation of the combination of Qiju Dihuang Pill and sodium hyaluronate eye drops for dry eye after cataract surgery

目的观察杞菊地黄丸联合玻璃酸钠滴眼液治疗白内障术后干眼的有效性和安全性。方法选取2019年1月—2019年12月符合纳入标准的白内障术后干眼患者72例(94只眼),随机分为观察组36例(46只眼)和对照组36例(48只眼)。对照组予0.1%玻璃酸钠滴眼液治疗,观察组在对照组基础上联合杞菊地黄丸口服,2组均治疗3个月。记录2组患者泪膜破裂时间(BUT)、基础泪液分泌试验(SIT)、角膜荧光素染色(FL)评分的数据及不良反应发生情况,并进行统计学分析。结果(1)BUT:与治疗前比较,观察组治疗后1、3个月均延长,差异均有统计学意义(t_(1个月)=5.219,t_(3个月)=13.754,均P=0.000)。对照组治疗后1、3个月均延长,差异均有统计学意义(t_(1个月)=4.591,t_(3个月)=14.605,均P=0.000)。治疗后3个月2组比较,观察组较对照组延长,差异有统计学意义(t=3.016,P=0.003)。(2)SIT:与治疗前比较,观察组治疗后1、3个月均增加,差异均有统计学意义(t_(1个月)=4.308,t_(3个月)=10.059,均P=0.000)。对照组治疗后1、3个月均增加,差异均有统计学意义(t_(1个月)=3.373,P=0.001;t_(3个月)=8.410,P=0.000)。治疗后3个月2组比较,观察组高于对照组,差异有统计学意义(t=2.712,P=0.008)。(3)FL:与治疗前比较,观察组治疗后1、3个月均降低,差异均有统计学意义(t_(1个月)=3.369,P=0.001;t_(3个月)=7.725,P=0.000)。对照组治疗后1、3个月均降低,差异均有统计学意义(t_(1个月)=3.376,P=0.001;t_(3个月)=5.814,P=0.000)。治疗后3个月2组比较,观察组较对照组降低,差异有统计学意义(t=2.566,P=0.012)。(4)总有效率:观察组总有效率为93.48%,高于对照组72.92%,差异有统计学意义(χ^(2)=7.031,P=0.008)。(5)不良反应:2组均未出现不良反应。结论杞菊地黄丸联合玻璃酸钠滴眼液治疗白内障术后干眼的远期疗效可观,且不良反应少。

OBJECTIVE To investigate the efficacy and safety of Qiju Dihuang Pill combined with sodium hyaluronate eye drops in treating dry eye after cataract surgery.METHODS A total of 72 cases patients(94 eyes)with dry eye who met the inclusion criteria after cataract surgery from January 2019 to December 2019 were selected and randomly divided into the observation group with 36 cases(46 eyes)and the control group with 36 cases(48 eyes).The control group was treated with 0.1%of sodium hyaluronate eye drops,and the observation group was additionally treated with Qiju Dihuang Pill based on the treatment of the control group.Both groups were treated for three months.The tear breakup time(BUT),Schirmer I test(SIT),fluorescent corneal staining(FL)scores,and the incidence of adverse reaction were observed and statistically analyzed.RESULTS(1)BUT:Compared with before treatment,at 1,3 months after treatment prolonged in both groups.The difference was statistically significant in observation group(t_(1 mon)=5.219,t_(3 mon)=13.754,P=0.000)and control group(t_(1 mon)=4.591,t_(3 mon)=14.605,P=0.000).Three months after treatment,BUT in the observation group was longer than that in the control group(t=3.016,P=0.003).(2)SIT:Compared with before treatment,SIT at 1 and 3 months after treatment increased markedly in both groups.The difference was statistically significant in observation group(t_(1 mon)=4.308,t_(3 mon)=10.059,P=0.000)and control group(t_(1 mon)=3.373,P=0.001,t_(3 mon)=8.410,P=0.000).Three months after treatment,SIT in the observation group was higher than in the control group(t=2.712,P=0.008).(3)FL:Compared with before treatment,FL scores at 1 and 3 months after treatment decreased markedly in both groups.The difference was statistically significant in observation group(t_(1 mon)=3.369,P=0.001,t_(3 mon)=7.725,P=0.000)and control group(t_(1 mon)=3.376,P=0.001,t_(3 mon)=5.814,P=0.000).Three months after treatment,FL scores in the observation group were lower than those in the control group(t=2.566,P=0.012).(4)Total effective rate:The total effective rate of the observation group(93.48%)was significantly higher than that of the control group(72.92%)(χ^(2)=7.031,P=0.008).(5)Adverse effect:Both groups had no adverse effects.CONCLUSIONS Qiju Dihuang Pill combined with sodium hyaluronate eye drops has a significant long-term effect in treating dry eye after cataract surgery without increasing the incidence of adverse effects.
