不同腧穴配伍治疗失眠模型大鼠效应机制差异的研究 被引量:1

摘要 目的通过辨证、辨病、辨病结合辨证选穴法对失眠模型大鼠血清中胃动素(MTL)、胃泌素(GAS)、5-羟色胺(5-HT)、褪黑素(MT)含量的影响,探讨不同腧穴配伍针刺治疗失眠效应机制的差异。方法 50只大鼠随机分为空白组(n=10)、模型组(n=10)、辨证选穴组(n=10)、辨病选穴组(n=10)、辨证结合辨病选穴组(n=10)。连续2 d腹腔注射PCPA混悬液(400 mg/kg)复制失眠大鼠模型。各治疗组给予针刺,每日1次,每次1 min,连续7 d。治疗7 d后对各组大鼠睡眠潜伏期(SL)和睡眠时间(ST)进行比较;ELISA法检测血清中MTL、GAS、5-HT、MT的含量。结果 与空白组对比,模型组睡眠潜伏期显著延长(P<0.01),睡眠时间明显缩短(P<0.01);与模型组对比,辨证、辨病、辨证结合辨病选穴组睡眠潜伏期缩短(P<0.05),睡眠时间延长(P<0.05),其中辨证结合辨证选穴组睡眠潜伏期显著缩短(P<0.01),睡眠时间明显延长(P<0.01)。与空白组对比,模型组血清中MTL、GAS、5-HT、MT含量显著降低(P<0.01);与模型组相比,辨证、辨病、辨证结合辨病选穴组血清中MTL、GAS、5-HT、MT含量增加(P<0.05),其中辨证选穴组血清中MTL、GAS含量增加明显(P<0.01),辨病结合辨证选穴组血清中5-HT、MT含量增加明显(P<0.01)。结论辨证、辨病、辨证结合辨病选穴针刺均可改善失眠大鼠睡眠质量,其效应机制可能与提高血清中MTL、GAS、5-HT、MT的含量有关,其中辨证选穴主要通过提高血清中MTL、GAS的含量改善睡眠;辨证结合辨病选穴及辨病选穴主要通过提高血清中5-HT、MT含量改善睡眠,结合延长睡眠时间、缩短睡眠潜伏期等依据,辨证结合辨病选穴优于辨证选穴及辨病选穴。 Objective To explore the effect mechanism difference in the insomnia acupuncture treatment among the different ways of acupoints compatibility by observing their influence on the contents of serum motilin (MTL),gastrin (GAS),5-hydroxytryptamine (5-HT),melatonin (MT) in insomnia model rats.Methods A total of fifty rats were randomly divided into a blank group (n=10),a model group (n=10),a syndrome differentiation and acupoint selection group (n=10),a disease differentiation and acupoint selection group (n=10),and a combined syndrome-disease differentiation and acupoint selection group (n=10).The insomnia rat model was established by an intraperitoneal injection of PCPA suspension (400 mg/kg) for two consecutive days.Each treatment group was treated with acupuncture,once a day,one minute each time,for 7 consecutive days.After 7 days of treatment,the sleep latency (SL) and sleep time (ST) of the rats in each group were compared,and the contents of MTL,GAS,5-HT and MT in serum were detected by ELISA.Results Compared with the blank group,the sleep latency of the model group was significantly prolonged (P<0.01),and the sleep time was significantly shortened (P<0.01).Compared with the model group,the sleep latency was shortened (P<0.05) and the sleep time was prolonged (P<0.05) in the three acupoint selection groups,among which the sleep latency was most significantly shortened (P<0.01) and the sleep time was most significantly prolonged (P<0.01) in the combined syndrome-disease differentiation and acupoint selection group.Compared with the blank group,the serum levels of MTL,GAS,5-HT and MT in the model group were significantly decreased (P<0.01).Compared with the model group,the contents of MTL,GAS,5-HT and MT in serum of the three acupoint selection groups were increased (P<0.05),among which the contents of MTL and GAS in serum of the syndrome differentiation and acupoint selection group were increased more significantly (P<0.01),and the serum 5-HT and MT contents in the combined syndrome-disease differentiation and acupoint selection group were increased more significantly (P<0.01).Conclusion The acupuncture treatment based on a syndrome differentiation,a disease differentiation,and a combined syndrome differentiation and disease differentiation can improve the sleep quality of insomnia rats,and their effect mechanisms may be related to the increase of serum MTL,GAS,5-HT and MT contents.Among them,the acupoint selection based on a syndrome differentiation can improve the sleep quality mainly by increasing the serum MTL and GAS contents,while the acupoint selection based on a combined syndrome-disease differentiation can improve the sleep quality mainly by increasing the contents of 5-HT and MT in serum.According to the effect in prolonging the sleep time and shortening the sleep latency,the acupoint selection treatment based on a combined syndrome-disease differentiation is proved to be better than that based on a syndrome differentiation or a disease differentiation.
