我国老年糖尿病患者衰弱影响因素的meta分析

Factors influencing frailty in elderly diabetes patients in China:a meta-analysis

目的通过meta分析探讨我国老年糖尿病患者发生衰弱的影响因素。方法计算机检索PubMed、Cochrane图书馆、EMbase、Web of Science、中国知网、万方数据库、中国生物医学文献数据库和维普中文期刊数据库中关于中国老年糖尿病患者衰弱影响因素的相关文献。检索时限为建库至2021年1月,采用Rev Man 5.3和Stata 14.0进行meta分析。结果共纳入11篇文献,累计老年糖尿病患者共4016例,其中衰弱人数共711例(17.70%)。meta分析结果显示,年龄(OR=1.16,95%CI:1.08~1.24)、糖化血红蛋白(OR=2.59,95%CI:1.18~5.70)、营养不良(OR=7.57,95%CI:6.51~8.81)、抑郁(OR=2.20,95%CI:1.47~3.30)、女性(OR=3.02,95%CI:1.68~5.42)、多病共存(OR=1.57,95%CI:1.35~1.82)、并发症(OR=3.49,95%CI:1.61~7.53)、认知(OR=2.14,95%CI:1.30~3.51)(P<0.05)是影响老年糖尿病患者衰弱发生的主要因素。结论老年糖尿病患者衰弱的发生与多种因素有关,应采取针对性的干预措施,预防老年衰弱的发生。

Objective To explore the factors affecting the occurrence of frailty among the elderly with diabetes in China through a meta-analysis.Methods We searched PubMed,Cochrane Library,EMbase,Web of Science,CNKI,Wanfang Database,CBM and VIP Chinese science and technology periodical full-text database to collect literatures regarding the factors influencing frailty among the elderly patients with diabetes in China and published from the establishment of the above-mentioned databases to January 2021.Rev Man 5.3 and Stata 14.0 software were used to perform a meta-analysis.Results A total of 11 articles were enrolled in this analysis,and there were accumulatively 4,016 elderly diabetic patients,of which 711(17.70%)patients were frailty.Meta-analysis results revealed that the main factors influencing the occurrence of frailty in the elderly diabetic patients were age(OR=1.16,95%CI:1.08-1.24),glycosylated hemoglobin(OR=2.59,95%CI:1.18-5.70),malnutrition(OR=7.57,95%CI:6.51-8.81),depression(OR=2.20,95%CI:1.47-3.30),female(OR=3.02,95%CI:1.68-5.42),coexistence of multiple diseases(OR=1.57,95%CI:1.35-1.82),complications(OR=3.49,95%CI:1.61-7.53),and cognition(OR=2.14,95%CI:1.30-3.51)(all P<0.05).Conclusion The occurrence of frailty in the elderly diabetic patients is related to multiple factors;and hence,targeted intervention measures should be taken to prevent the occurrence of frailty in the elderly.
