玻璃体内注射曲安奈德与康柏西普治疗白内障术后黄斑水肿的效果比较

Effects of intravitreal injection of tramanide versus compacseep on macular edema after cataract

目的比较玻璃体内注射曲安奈德与康柏西普治疗白内障术后黄斑水肿的临床效果。方法抽取2019年1月至2020年12月平煤神马医疗集团总医院收治的白内障术后黄斑水肿患者724例,按随机数字表法分为研究组(362例)与对照组(362例)。对照组采用玻璃体内注射曲安奈德,研究组采用玻璃体内注射康柏西普。对比两组治疗前后眼压、最佳矫正视力(BCVA)、黄斑中心凹厚度(CMT)及血清干扰素诱导蛋白-10(IP-10)、血管内皮生长因子(VEGF)水平。比较两组眼底情况和不良反应发生率。结果治疗后,两组眼压均较治疗前降低,且研究组低于对照组(P<0.05)。治疗后,两组BCVA均较治疗前增高,CMT均较治疗前降低,且研究组BCVA高于对照组,CMT低于对照组(P<0.05);研究组眼底情况优于对照组(P<0.05)。治疗后,两组血清IP-10、VEGF水平均较治疗前降低,且研究组低于对照组(P<0.05)。研究组不良反应发生率为3.04%(11/362),低于对照组的6.35%(23/362),P<0.05。结论与玻璃体内注射曲安奈德相比,康柏西普治疗白内障术后黄斑水肿可显著改善眼压、CMT和眼底情况,调节血清IP-10、VEGF表达,提高视功能和视力水平,且不良反应发生率低。

Objective To compare and analyze the clinical effects of intravitreal injection of triamcinolone acetonide versus cambacip in the treatment of macular edema after cataract surgery.Methods A total of 724 patients with macular edema after cataract surgery treated in General Hospital of Pingmei Shenma Group from January 2019 to December 2020 were selected.According to random number table method,they were divided into study group(362 cases)and control group(362 cases).The control group received intravitreal injection of triamcinolone acetonide,and the study group received intravitreal injection of compaxil.Intraocular pressure,best corrected visual acuity(BCVA),macular fovea thickness(CMT),serum interferon-induced protein-10(IP-10)and vascular endothelial growth factor(VEGF)were compared between the two groups before and after treatment.And the conditions of fundus and the incidence of adverse reactions were compared between the two groups.Results After treatment,the intraocular pressure of the two groups were lower than those before treatment,and the study group had lower intraocular pressure than the control group(P<0.05).After treatment,BCVA increased,and CMT decreased in the two groups,compared with before treatment;and postoperative BCVA in the study group was higher than that in the control group,while postoperative CMT was lower than that in the control group(P<0.05).The fundus condition of the study group was better than that of the control group(P<0.05).After treatment,the levels of serum IP-10 and VEGF in the two groups decreased,compared with those before treatment;and the levels of serum IP-10 and VEGF of study group were lower than those of the control group(P<0.05).The incidence of adverse reactions in the study group was 3.04%(11/362),lower than 6.35%(23/362)in the control group(P<0.05).Conclusions Compared with intravitreal injection of triamcinolone acetonide,compacseil in the treatment of macular edema after cataract surgery can significantly improve intraocular pressure,CMT,and fundus,regulate the expression of serum IP-10 and VEGF,improve visual function and visual acuity,with low incidence of adverse reactions.
