摘要
目的:探讨步行运动在射血分数保留的心力衰竭(HFpEF)患者中的应用效果。方法:选择120例在我院住院的HFpEF患者为研究对象,随机分为常规治疗组和步行运动组(在常规治疗基础上进行规律步行运动),各60例,治疗24周后比较两组治疗前后左房容积指数(LAVI)、6min步行距离(6MWD)、N末端脑钠肽前体(NT-proBNP)水平、明尼苏达州心力衰竭生活质量评分表(MLHFQ)得分的变化情况。结果:与治疗前比较,治疗24周后两组患者LAVI、NT-proBNP水平均显著下降,6MWD均显著增加,P均=0.001;与常规治疗组比较,步行运动组LAVI[(39.62±8.54)ml/m^(2)比(30.54±10.21)ml/m^(2)]、NT-proBNP[(438.31±106.83)pg/ml比(284.62±101.43)pg/ml]水平及MLHFQ[(40.81±8.57)分比(25.42±7.64)分]评分均显著下降,6MWD[(378.93±112.55)m比(484.61±110.43)m]显著增加,P均=0.001。结论:对于HFpEF患者步行运动可显著改善左室舒张功能,显著增加运动功能,并显著提高生活质量,值得推广。
Objective:To explore application effect of walking exercise in patients with heart failure with preserved ejection fraction(HFpEF).Methods:A total of 120 HFpEF inpatients from our hospital were randomly and equally divided into routine treatment group and walking exercise group(received regular walking exercise based on routine treatment),both groups were treated for 24 weeks.Left atrial volume index(LAVI),6min walking distance(6MWD),level of N-terminal pro brain natriuretic peptide(NT-proBNP),score of Minnesota living with heart failure questionnaire(MLHFQ)before and after treatment were compared between two groups.Results:Compared with before treatment,after 24-week treatment,there were significant reductions in LAVI and NT-proBNP level,and significant rise in 6MWD in two groups,P=0.001 all;compared with routine treatment group,there were significant reductions in LAVI[(39.62±8.54)ml/m^(2)vs.(30.54±10.21)ml/m^(2)],NT-proBNP level[(438.31±106.83)pg/ml vs.(284.62±101.43)pg/ml]and MLHFQ score[(40.81±8.57)scores vs.(25.42±7.64)scores],and significant rise in 6MWD[(378.93±112.55)m vs.(484.61±110.43)m]in walking exercise group,P=0.001 all.Conclusion:Walking exercise can significantly improve left ventricular diastolic function,motor function and quality of life in HFpEF patients,which is worth extending.
