推拿治疗2型糖尿病的临床选穴规律

Clinical Acupoint Selection Rules in Massage Therapy in Treatment of Type 2 Diabetes

目的:运用数据挖掘技术,分析推拿治疗2型糖尿病的选穴规律,为临床治疗提供参考依据。方法:选择中国期刊全文数据库、万方数据库、维普数据库、中国生物医学文献服务系统、PubMed、Medline以及Embase数据库中推拿治疗2型糖尿病的临床文献并进行筛选,利用Microsoft Office Excel 2019整理纳入文献的推拿选穴处方,进行选穴频数统计筛选高频腧穴;再将高频腧穴数据导入SPSS Modeler 18.0软件建立数据库,运用Apriori算法进行关联分析,筛选最佳推拿腧穴组合,再将高频腧穴数据导入SPSS Statistic 25.0,进行聚类分析,筛选出最佳推拿腧穴处方。结果:对纳入的45篇文章进行分析,推拿治疗2型糖尿病的主要腧穴有足三里、脾俞、肾俞、腹、胃脘下俞,足太阳膀胱经,任脉,足阳明胃经腧穴使用频率最高,穴位选取集中在腰背部和下肢部,分析高频腧穴得出33条关联规则。通过聚类分析将高频腧穴聚成3类,总结为3个腧穴处方,分别对应"中满内热""脏腑柔弱""卫分郁热"型的2型糖尿病患者。结论:推拿治疗2型糖尿病选穴具有一定的规律性,选穴以腰背部为主,侧重足太阳膀胱经取穴。

Objective: To analyze the rules of acupoint selection in massage therapy for type 2 diabetes based on data mining technology, so as to provide reference for clinical treatment.Methods: The clinical literatures on massage in the treatment of type 2 diabetes from China National Knowledge Internet(CNKI),Wanfang Database, VIP Database(VIP),China Biomedical Literature Service System(SinoMed),Pubmed, Medline and Embase Database were searched and selected.Microsoft Office Excel 2019 was used to sort out the massage acupoint selection prescriptions included in the literature, and the high-frequency acupoint selection frequency was statistically screened.Then the high frequency acupoint data was imported into SPSS Modeler 18.0 software to establish a database, and the Apriori algorithm was used for correlation analysis to screen the best combination of acupoint massage.Then the high frequency acupoint data was imported into SPSS Statistic 25.0 for cluster analysis to screen out the best acupoint prescription of massage.Results: Based on the analysis of the included 45 articles, the main acupoints for the treatment of type 2 diabetes by massage were Zusanli(ST36),Pishu(BL20),Shenshu(BL23),abdomen, Weiwanxiashu(EX-B3).Acupoints on Taiyang Bladder Meridian of Foot, Ren Meridian and Yangming Stomach Meridian of Foot were the most frequently used.The selection of acupuncture points focused on the lower back and lower limbs.Thirty-three association rules were obtained by analyzing the high-frequency acupoints.Through cluster analysis, the high-frequency acupoints were grouped into 3 categories, and they were summarized into three acupoint prescriptions, corresponding to type 2 diabetic patients with "abdomen distension and internal heat", "visceral weakness",and "stagnation heat in Weifen".Conclusion: it is believed that massage therapy has certain regularity in selecting acupoints for type 2 diabetes.The selection of acupoints is mainly on the lower back, focusing on the acupoint selection of the Taiyang Bladder Meridian of Foot.
