无创呼吸机对COPD伴呼吸衰竭患者血气分析指标、STREM-1及HMGB-1水平的影响 被引量:7

摘要 目的探讨无创呼吸机对慢性阻塞性肺疾病(COPD)伴呼吸衰竭患者血气分析指标、可溶性髓样细胞触发受体-1(STREM-1)及高迁移率族蛋白B1(HMGB-1)水平的影响。方法选取2020年1月至2021年1月我院收治的80例COPD伴呼吸衰竭患者作为研究对象,按照电脑数字表法将其随机分为对照组与试验组,各40例。对照组给予常规治疗,试验组在对照组基础上给予无创呼吸机治疗。比较两组患者治疗前、后的血气分析指标、STREM-1、HMGB-1水平及病情改善情况。结果治疗后,两组的PaCO;均降低,PaO;均升高,且试验组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的STREM-1、HMGB-1水平均降低,且试验组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的6 min步行距离均增加,SGRQ评分均降低,且试验组优于对照组,差异具有统计学意义(P<0.05)。结论无创呼吸机可有效改善COPD伴呼吸衰竭患者的血气分析指标,并降低STREM-1、HMGB-1水平,进而有效改善患者的病情。 Objective To investigate the effects of non-invasive ventilator on blood gas analysis indexes, soluble triggering receptor expressed on myeloid cell-1(STREM-1) and high-mobility group box 1(HMGB-1) levels of patients with chronic obstructive pulmonary disease(COPD) complicated with respiratory failure. Methods A total of 80 patients with COPD complicated with respiratory failure treated in our hospital from January 2020 to January 2021 were selected as the research objects. The patients were randomly divided into control group and experimental group according to the computer digital table method, with 40 cases in each group. The control group was given routine treatment, and the experimental group was given non-invasive ventilator treatment on the basis of the control group. The blood gas analysis indexes, STREM-1, HMGB-1 levels and condition improvement before and after treatment were compared between the two groups. Results After treatment, PaCO;in the two groups decreased, PaO;increased, those in the experimental group were better than the control group, and the differences were statistically significant(P<0.05). After treatment, the levels of STREM-1 and HMGB-1 decreased in the two groups, those in the experimental group were lower than the control group, and the differences were statistically significant(P<0.05). After treatment, the 6 min walking distance increased in the two groups, the SGRQ score decreased, those in the experimental group were better than the control group, and the differences were statistically significant(P<0.05). Conclusion Non-invasive ventilator can effectively ameliorate the blood gas analysis indexes of patients with COPD complicated with respiratory failure, reduce the levels of STREM-1 and HMGB-1, and then effectively improve the condition of patients.
