摘要 目的探讨大剂量布地奈德混悬液雾化吸入治疗哮喘急性发作的临床效果,以为该病治疗方式的选择提供参考。方法选取2020年4月至2021年4月我院收治的146例哮喘急性发作患儿作为研究对象,采取抽签法将其分为参照组(73例,常规治疗+小剂量布地奈德混悬液雾化吸入+孟鲁司特钠)和试验组(73例,常规治疗+大剂量布地奈德混悬液雾化吸入+孟鲁司特钠)。比较两组的临床疗效及治疗前、后的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)水平与Nod样受体热蛋白结构域相关蛋白3(NLRP3)mRNA。结果试验组的治疗总有效率为97.26%,高于参照组的87.67%,差异具有统计学意义(P<0.05)。治疗前,两组的IL-1β、IL-6水平及NLRP3 mRNA比较,差异无统计学意义(P>0.05);治疗后,两组的IL-1β、IL-6水平及NLRP3 mRNA均较治疗前降低,且试验组低于参照组,差异具有统计学意义(P<0.05)。结论大剂量布地奈德混悬液雾化吸入可提升哮喘急性发作患儿的临床疗效,降低IL-1β、IL-6水平及NLRP3 mRNA,具有一定的临床应用和推广价值。 Objective To explore the clinical effect of high-dose budesonide suspension atomization inhalation in the treatment of acute attack of asthma, so as to provide reference for the selection of treatment methods of this disease.Methods A total of 146 children with acute attack of asthma treated in our hospital from April 2020 to April 2021 were selected as the research objects, the children were divided into reference group(73 cases, routine treatment + low-dose budesonide suspension atomization inhalation + montelukast sodium) and experimental group(73 cases, routine treatment+ high-dose budesonide suspension atomization inhalation + montelukast sodium). The clinical efficacy, interleukin-1β(IL-1β), interleukin-6(IL-6) levels and Nod-like receptor pyrin domain-containing protein 3(NLRP3) mRNA before and after treatment were compared between the two groups. Results The total effective rate of treatment in the experimental group was 97.26%, which was higher than 87.67% in the reference group, and the difference was statistically significant(P<0.05). Before treatment, There were no significant differences in the IL-1β, IL-6 levels and NLRP3 m RNA between the two groups(P>0.05);after treatment, the IL-1β, IL-6 levels and NLRP3 mRNA in the two groups were lower than those before treatment, and those in the experimental group were lower than the reference group,and the differences were statistically significant(P <0.05). Conclusion High-dose budesonide suspension atomization inhalation can improve the clinical efficacy of children with acute attack of asthma, reduce the IL-1β, IL-6 levels and NLRP3 m RNA, with certain clinical application and popularization value.
