揿针配合熏蒸治疗痰热郁肺型慢性阻塞性肺疾病疗效观察

Efficacy Observation of Intradermal Needles Combined with Fumigation for Chronic Obstructive Pulmonary Disease Due to Phlegm Heat Obstructing the Lung

目的观察揿针配合中药熏蒸治疗痰热郁肺型慢性阻塞性肺疾病(COPD)的临床疗效及其对患者肺功能的改善作用。方法将96例痰热郁肺型COPD患者随机分为治疗组和对照组,每组48例。对照组采用常规药物治疗,治疗组在对照组基础上采用揿针配合中药熏蒸治疗。观察两组治疗前后中医证候评分、各项血清炎症因子[C-反应蛋白(CRP)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]水平、肺功能[第1秒用力呼气量(FEV1)、FEV1/用力肺活量百分比(FEV1/FVC)]指标、6 min步行距离(6MWD)及各项生活质量评分(疾病症状评分、活动能力评分、疾病影响评分)的变化情况,并比较两组临床疗效。结果治疗组总有效率为93.8%,对照组为70.8%,两组比较差异具有统计学意义(P<0.05)。两组治疗后中医证候评分、各项血清炎症因子水平、肺功能指标、6MWD及各项生活质量评分均较同组治疗前改善(P<0.05)。治疗组治疗后中医证候评分、各项血清炎症因子水平、肺功能指标、6MWD及各项生活质量评分与对照组比较,差异均具有统计学意义(P<0.05)。结论揿针配合中药熏蒸治疗痰热郁肺型COPD疗效确切,可降低气道炎症,改善患者临床症状及肺功能。

Objective To observe the clinical efficacy of intradermal needles plus Chinese medicinal fumigation in treating chronic obstructive pulmonary disease(COPD)due to phlegm-heat obstructing the lung and its effect in improving patients’pulmonary function.Method Ninety-six patients with COPD due to phlegm-heat obstructing the lung were randomized into a treatment group and a control group,with 48 cases in each group.The control group received conventional medications for treatment,and the treatment group was given additional intradermal-needle and Chinese medicinal fumigation therapies.Before and after the treatment,the traditional Chinese medicine(TCM)symptom score,serum inflammatory factor levels[C-reactive protein(CRP),interleukin(IL)-8,and tumor necrosis factor-α(TNF-α)],pulmonary function indicators[forced expiratory volume in 1 second(FEV1)and the ratio of FEV1 to the forced vital capacity(FEV1/FVC)],6 min walking distance(6MWD),and quality of life(QOL)rating scores(scores of the disease symptoms,activities,and disease impact)were observed for the two groups.The clinical efficacy was also compared.Result The total effective rate was 93.8%in the treatment group versus 70.8%in the control group,and the between-group difference was statistically significant(P<0.05).The TCM symptom score,serum inflammatory factor levels,pulmonary function indicators,6MWD,and QOL rating scores improved after the intervention in both groups(P<0.05).After the intervention,there were significant differences in the TCM symptom score,serum inflammatory factor levels,pulmonary function indicators,6 MWD,and QOL rating scores between the two groups(P<0.05).Conclusion Intradermal needles plus Chinese medicinal fumigation is effective in treating COPD due to phlegm-heat obstructing the lung;it can reduce airway inflammation and improve the clinical symptoms and pulmonary function.
