基于医院信息系统的随访系统在耳鸣远期疗效跟踪管理研究

摘要 目的:研究基于医院信息系统(HIS)的随访系统在耳鸣远期疗效跟踪随访管理中的应用。方法:设计基于HIS的随访管理系统,搭建随访系统管理平台。选取医院收治的120例耳鸣患者临床资料,按照不同的跟踪随访管理方法将其分为对照组与观察组,每组60例,对照组实施常规跟踪随访管理,观察组采用基于HIS的随访系统进行跟踪随访管理,对比两组患者随访6个月期间对疾病相关健康知识的掌握情况、随访依从率、耳鸣致残量表(THI)评分视觉模拟量表(VAS)评分、耳鸣响度(TI)以及医护人员对两组管理方法的满意度。结果:观察组患者对合理饮食、合理用药、保持清洁卫生、戒烟限酒、定期复查、规律生活和心理稳定知识掌握的情况好于对照组,差异有统计学意义(χ^(2)=6.508,χ^(2)=4.357,χ^(2)=6.708,χ^(2)=4.093,χ^(2)=3.871,χ^(2)=4.093,χ^(2)=4.357;P<0.05)。观察组随访3个月、随访6个月的依从率均高于对照组,两组比较差异有统计学意义(χ^(2)=8.076,χ^(2)=8.044;P<0.05)。观察组随访6个月时THI、VAS和TI评分均低于对照组,差异有统计学意义(t=23.530,t=8.050,t=5.852;P<0.05)。观察组与对照组相比医护人员认为从系统获益、认为具有便携性,以及对系统的安全性、时效性及稳定性的满意度较高,差异有统计学意义(Fisher精确概率检验,P<0.05)。结论:基于HIS的随访系统应用于耳鸣患者的远期跟踪随访管理中可显著提高患者对疾病相关知识的掌握情况,提高患者对随访工作的依从率及干预治疗效果,并可提高医护人员对该系统实施的跟踪随访管理的满意度。 Objective:To explore and analyze the application of the follow-up system based on hospital information system(HIS)in the follow-up management of long-term curative effect of tinnitus.Methods:The follow-up management system based on HIS was designed,and a management platform of follow-up system was established.The clinical data of 120 patients with tinnitus who admitted to hospital were selected and were divided into control group and observation group according to different follow-up management methods,with 60 cases in each group.The control group implemented routine follow-up management,and the observation group adopted follow-up system based on HIS for follow-up management.The mastery of disease-related health knowledge,follow-up compliance rate,tinnitus disability scale score(THI),visual analog scale(VAS)score,tinnitus loudness(TI)and the satisfaction of medical care personnel for management method during 6-month follow-up period of two groups were compared.Results:The masteries of patients’knowledge for reasonable diet,rational drug use,keeping clean and hygienic,quitting smoking and limiting alcohol,regular review,regular life and psychological stability in observation group were significantly better than those in control group(χ^(2)=6.508,χ^(2)=4.357,χ^(2)=6.708,χ^(2)=4.093,χ^(2)=3.871,χ^(2)=4.093,χ^(2)=4.357,P<0.05).The compliance rates after 3-month follow-up and after 6-month follow-up in observation group were significantly higher than those in control group(χ^(2)=8.076,χ^(2)=8.044,P<0.05),respectively.The scores of THI,VAS and TI post 6-month follow-up in observation group were significantly lower than those in control group(t=23.530,t=8.050,t=5.852,P<0.05).The proportions of medical care personnel,who took for obtaining benefit from system and with portability,and satisfactions of them for safety,timeliness and stability in observation group were significantly higher than those of control group,and the differences of them between two groups were significant by Fisher exact probability(P<0.05).Conclusion:The application of this follow-up system based on HIS in the long-term follow-up management of patients with tinnitus can significantly improve the grasp of patients for disease-related knowledge,and enhance the compliance rate of patients for follow-up work and the effect of intervention treatment.It can enhance the satisfaction of medical care personnel for follow-up management that was implemented by this system.
