Er:YAG+Nd:YAG激光联合治疗慢性牙周炎的疗效研究

Study of Er:YAG laser combine with Nd:YAG laser in the treatment of chronic periodontitis

目的分别采用超声+手工牙周治疗和Er:YAG+Nd:YAG激光联合辅助治疗慢性牙周炎患者,对比两种治疗前后的临床指标,探讨Er:YAG+Nd:YAG激光联合治疗的效果。方法选取2018年09月-2020年05月于厦门医学院附属口腔医院就诊的80例慢性牙周炎患者,采取随机半口对照设计方案,试验组采用Er:YAG+Nd:YAG激光治疗,对照组采用超声+手工龈下刮治、根面平整,分别在术前、术后1个月、3个月、6个月、12个月记录牙周指数(探诊深度、临床附着水平、探诊出血、菌斑指数、牙龈指数),统计数据。结果试验组GI在1个月、3个月时较对照组降低(P_(1)=0.029,P_(2)=0.049),两组间PI、BOP%在12个月内没有显著差异。PD=3-6mm时两组PD、CAL在治疗后12个月内没有统计学差异,但当PD>6mm时,试验组PD、CAL在治疗后1个月、3个月、6个月时较对照组降低(P_(1)=0.029,P_(2)=0.023,P_(3)=0.006;P_(4)=0.034,P_(5)=0.029,P_(6)=0.011)。结论相比SRP,联合应用Er:YAG、Nd:YAG激光辅助治疗慢性牙周炎患者在短期内更利于牙龈炎症的恢复,能显著降低深牙周袋患牙的探诊深度和临床附着水平。

Objective Compared the effect of combined Er:YAG and Nd:YAG laser therapy to that of mechanical scaling and root planing in non-surgical treatment of chronic periodontitis.Methods Selected 80 patients with chronic periodontitis,The quadrants were randomly allocated in a splitmouth design to either combined Er:YAG and Nd:YAG laser therapy or mechanical scaling and root planing,respectively in the preoperative and postoperative 1 month,3 months record periodontal index(probing depth,clinical attachment level,bleeding on probing,plaque index,gingival index),and statistical data.Results GI in the experimental group was lower than that in the control group at 1 month,3 months(P1=0.029,P2=0.049),but there was no significant difference in PI and BOP%between the two groups.When PD=3-6mm,there was no statistical difference between the two groups before and after treatment of PD and CAL.However,when PD>6mm,PD and CALof the experimental group was lower than that of the control group at1,3,6 months after treatment(P1=0.029,P2=0.023,P3=0.006;P4=0.034,P5=0.029,P6=0.011).Conclusion Compared with SRP,the combined application of Er:YAG laser and Nd:YAG laser in the treatment of chronic periodontitis is more conducive to the recovery of gingival inflammation in the short term,and can significantly reduce the depth of exploration and clinical attachment level of deep periodontal pocket affected teeth.
