基于数据挖掘关幼波教授辨治肝病的证治规律

Discussion on the law of syndrome and treatment of professor Guan Youbo in treating liver disease based on data mining

目的:基于异病同治的中医治则,挖掘关幼波教授在肝病诊治中的辨证及遣方用药特点,探索关幼波教授辨治肝病的证治规律。方法:收集关幼波教授治疗肝病的医案,运用古今医案云平台(V2.2.3)软件进行统计描述及关联分析等,总结关幼波教授辨治肝病的常见证候及相应治法、用药等规律。结果:关幼波教授辨治肝病的病位主要在肝、脾、胆、肾,病性以虚、湿、热、瘀为主;常见中医证候有肝胆湿热证、气虚血滞证、气血两虚证、肝郁血滞证、阴虚血热证、肝肾阴虚证等;治法有清热利湿、活血化痰、补气养血、活血解毒、舒肝解毒、滋补肝肾等;关联分析所得常用药对8对、中医证候-中药组合10类。结论:关幼波教授辨治肝病重视正邪关系,多从湿、热、瘀、虚着手,辨证时主张十纲辨证、正邪兼顾,治疗上重视调理气血、中州当先、活血化痰等。

Objective:Based on the principle of treating different diseases with the same treatment in traditional Chinese medicine,to explore the characteristics of Professor Guan Youbo's syndrome differentiation and prescriptions in liver disease diagnosis and treatment,and to explore the rules of Professor Guan Youbo's syndrome differentiation and treatment of liver diseases.Methods:Collect the medical records of Professor Guan Youbo's treatment of liver disease,use the ancient and modern medical record cloud platform(V2.2.3)software to perform statistical description and correlation analysis,etc.,summarize the common syndromes of Professor Guan Youbo's treatment of liver disease and the corresponding laws of treatment and medication.Results:Professor Guan Youbo's liver disease diagnosis and treatment were mainly in the liver,spleen,gallbladder,and kidneys.The main diseases were deficiency,dampness,heat,andblood stasis.Common TCM syndromes include liver and gallbladder damp-heat syndrome,qi deficiency and blood stagnation syndrome,and qi and blood.Deficiency syndrome,liver stagnation and blood stagnation syndrome,yin deficiency and blood heat syndrome,liver and kidney yin deficiency syndrome,etc;treatment methods include clearing heat and removing dampness,promoting blood circulation to resolve phlegm,nourishing qi and nourishing blood,promoting blood circulation and detoxification,soothing liver and detoxification,nourishing liver and kidney,etc;The correlation analysis obtained 8 pairs of commonly used medicines,and 10 types of TCM syndromes and TCM combinations.Conclusion:Professor Guan Youbo pays attention to the relationship between right and evil in differentiating and treating liver diseases,mostly starting from dampness,heat,blood stasis,and deficiency.He advocates differentiation of symptoms and signs of ten principles and taking both right and evil into account during syndrome differentiation.In treatment,he emphasizes regulating qi and blood,leading Zhongzhou,promoting blood circulation and eliminating phlegm.
