2型糖尿病患者低血糖恐惧与自我效能、生存质量的相关性研究

Correlation of Hypoglycemia Fear with Self-efficacy and Quality of Life in Patients with Type 2 Diabetes Mellitus

目的:调查2型糖尿病患者低血糖恐惧与生存质量、自我效能的现状以及分析其相关性,为2型糖尿病患者的疾病控制提供依据。方法:采用便利抽样法,于2018年6月-11月选取在某市某三级甲等综合医院内分泌科住院部就诊的糖尿病患者220例作为研究对象,调查其一般资料、低血糖恐惧、生存质量及自我效能现状,并使用斯皮尔曼等级相关分析其相关关系。结果:研究中175名2型糖尿病患者低血糖恐惧得分范围是0~49分,中位数与四分位数间距是[15(12,19)];自我效能得分范围是2.13~13.38分,中位数与四分位数间距是[8.125(6.88,9.00)];生存质量得分范围是29~112分,中位数与四分位数间距是[58(48,70)]。2型糖尿病患者低血糖恐惧与生存质量成正相关(r=0.225,P<0.01),与自我效能无明确相关关系。结论:2型糖尿病患者具有高水平的低血糖恐惧和较差的生存质量。应加强对低血糖患者的健康教育,设计针对性干预措施以降低患者低血糖恐惧,改善其生存质量。

Objective:To investigate the status of hypoglycemia fear,quality of life and self-efficacy in patients with type 2 diabetes mellitus and analyze their correlation,so as to provide evidence for disease control in patients with type 2 diabetes.Methods:A total of 220 diabetic patients in the inpatient department of endocrinology in a tertiary general hospital from June to November 2018 were selected as the research objects by convenience sampling method.The general information,hypoglycemia fear,quality of life and self-efficacy were investigated,and the correlation between them was analyzed by Spearman rank correlation.Results:The score range of hypoglycemia fear in 175 patients with type 2 diabetes mellitus was 0-49,and the median and quartile spacing was[15(12,19)];the score range of self-efficacy was 2.13-13.38,the median and quartile spacing was[8.13(6.88,9.0008)];the score range of quality of life was 29-112,and the median and quartile spacing was[58(48,70)].There was a positive correlation between hypoglycemia fear and quality of life in type 2 diabetic patients(r≤0.225,P<0.01),but there was no clear correlation between fearofhypoglycemic and self-efficacy.Conclusion:Patients with type 2 diabetes have a high level of fear of hypoglycemia and poor quality of life.Health education on hypoglycemia events should be strengthened,and targeted interventions should be designed to reduce patients'fear of hypoglycemia and improve their quality of life.
