新诊断2型糖尿病伴肥胖患者五种血清脂肪因子水平和腹部脂肪水平变化及相关性分析 被引量:6

The Relationship between Five Adipocytokines and Abdominal Adipose Tissue in Obesity Patients with Newly Diagnosed Type 2 Diabetes Mellitus

摘要 目的:探讨新诊断2型糖尿病(T2DM)患者五种血清脂肪因子水平与腹部脂肪之间的相关性。方法:连续性纳入2019-06-2020-12于我院门诊或住院发现的108例肥胖新诊断T2DM患者,70例年龄和体重相符的非糖尿病体检患者作为对照组。所有纳入受检者均检测血清丝氨酸蛋白酶抑制剂(VAS)、脂联素(APN)、脂肪素(APE)、白脂素(ASP)和网膜素-1(OMEN)五种经典的脂肪因子水平,腹部超声评价内脏脂肪组织(VAT)和皮下脂肪组织(SAT),并计算内脏脂肪指数(VAI)。比较两组对象以上各指标的差异,并分析新诊断T2DM患者VAT与血清脂肪因子、SAT、VAI的相关性。结果:新诊断T2DM组患者糖化血红蛋白(HbA1c)、空腹血糖和胰岛素抵抗指数(HOMA-IR)均明显高于对照组(P<0.05)。新诊断T2DM组患者APN和OMEN水平均明显低于对照组(P<0.01),而VAS、APE和ASP水平明显高于对照组(P<0.01)。相关性分析提示VAT与APE的相关系数最大(r=0.412,P<0.01;r_(调整年龄)=0.425,P<0.01;r_(调整年龄+BMI)=0.372,P<0.01)。结论:新诊断T2DM伴肥胖患者血清脂肪因子水平和VAT水平均明显升高,其中脂肪因子APE与VAT最为相关。 Objective:To evaluate the relationship between five adipocytokines and abdominal adipose tissue in obesity patients with newly diagnosed type 2 diabetes mellitus(T2DM).Method:One-hundred and eight obesity patients with newly diagnosed T2DM and seventy age-and-weight matched controls without non-diabetes mellitus were enroll in this study.All the patients were tested the levels of vaspin(VAS),adiponectin(APN),apelin-13(APE),asprosin(ASP)and omentin-1(OMEN).Abdomen ultrasound was used to measured visceral adipose tissue(VAT)and subcutaneous adipose tissue(SAT).Results:The levels of HbA1c,fast glucose and HOMA-IR of newly diagnosed T2DM group were higher than those of control group(P<0.05).The levels of APN and OMEN in newly diagnosed T2DM group were lower than those in control group(P<0.01),while levels of VAS,APE and ASP were higher than those in control group(P<0.01).Correlation analysis showed that VAT was most closely related to APE(r=0.412,r_(age adjusted)=0.425,r_(age+BMI adjusted)=0.372,P<0.01).Conclusion:The levels of adipocytokines and VAT are elevated in obesity patients with newly diagnosed T2DM.APE is most closely related to VAT.
