摘要
目的探讨妊娠合并心脏病患者基质金属蛋白酶-1(MMP-1)、GATA结合蛋白6(GATA6)、正五聚蛋白3(PTX3)与不良妊娠结局的关系。方法选取2019年1月至2021年1月温州市中西医结合医院收治的妊娠合并心脏病患者42例,根据是否发生不良妊娠结局分为观察组(20例)和对照组(22例),比较两组的妊娠风险及MMP-1、GATA6 mRNA和PTX3水平,分析MMP-1、GATA6 mRNA和PTX3与妊娠风险分级关系及预测不良妊娠结局的价值。结果观察组妊娠风险分级与对照组比较,差异有统计学意义(P<0.05)。观察组MMP-1、PTX3高于对照组,GATA6 mRNA低于对照组,差异均有统计学意义(均P<0.05);MMP-1和PTX3与妊娠风险分级呈正相关(r=0.598、0.829,均P<0.05),GATA6 mRNA与妊娠风险分级呈负相关(r=-0.690,P<0.05)。多因素logistic回归分析显示MMP-1、GATA6 mRNA和PTX3是不良妊娠结局的相关影响因素(均P<0.01);联合MMP-1、GATA6 mRNA和PTX3预测不良妊娠结局的AUC大于MMP-1、GATA6 mRNA或PTX3单一预测。结论MMP-1、GATA6和PTX3与妊娠合并心脏病患者不良妊娠结局有关。
Objective To investigate the relationship between matrix metalloproteinase-1(MMP-1),GATA binding protein 6(GATA6)and pentraxin 3(PTX3)and adverse pregnancy outcomes in pregnant women complicated by heart disease.Methods Forty-two pregnant women with heart disease admitted to Wenzhou Integrated Traditional Chinese and Western Medicine Hospital from January 2019 to January 2021 were enrolled and divided into obersavation group(20 cases)and control group(22 cases)according to pregnancy outcomes.Pregnancy risk,MMP-1,GATA6 mRNA and PTX3 were compared between the two groups.The relationship between MMP-1,GATA6 mRNA and PTX3 and pregnancy risk classification were analyzed.The value of MMP-1,GATA6 mRNA and PTX3 in predicting adverse pregnancy outcomes was determined.Results The pregnancy risk classification was significantly higher in obersavation group than in control group(P<0.05).MMP-1 and PTX3 were significantly higher and GATA6 mRNA was significantly lower in obersavation group than in control group(all P<0.05).MMP-1 and PTX3 were positively correlated with pregnancy risk classification(r=0.598,0.829,all P<0.05),and GATA6 mRNA was negatively correlated with pregnancy risk classification(r=-0.690,all P<0.05).Multivariate logistic regression analysis showed MMP-1,GATA6 mRNA and PTX3 were influence factors of adverse pregnancy outcome(all P<0.01).The area under the curve for predicting adverse pregnancy outcome was significantly higher using a combination of MMP-1,GATA6 mRNA and PTX3 than using MMP-1,GATA6 mRNA or PTX3 alone.Conclusion MMP-1,GATA6 and PTX3 are related to adverse pregnancy outcomes in pregnant women with heart disease.
