集束化护理对重症肺炎新生儿血气分析指标及机械通气时间的影响

Effect of Intensive Nursing on Blood Bas Analysis and Mechanical Ventilation Time in Neonates with Severe Pneumonia

目的评价集束化护理的应用对重症肺炎新生儿血气分析指标、机械通气时间的影响,通过提高护理质量促进重症肺炎新生儿预后。方法选择本院2018年2月至2019年7月收治住院治疗的重症肺炎新生儿92例,采用随机数表法进行分组,各46例。两组均予以对症治疗,对照组同时配合常规护理,干预组在对照组基础上配合集束化护理干预。对比对照组与干预组治疗效果,包括血气分析指标改善情况、机械通气时间、咳嗽消失时间、哮鸣音消失时间以及家属护理满意度。结果干预组机械通气时间、咳嗽消失时间、哮鸣音消失时间均明显短于对照组,P<0.05;干预组重症肺炎新生儿家属护理满意度明显高于对照组,P<0.05;两组患儿干预后pH、血氧饱和度(SpO2)、动脉血氧分压(PaO2)水平较干预前改善,且干预组患儿的血气分析指标改善情况更明显,P<0.05。结论重症肺炎新生儿对症支持治疗的同时配合集束化护理干预的效果显著,可进一步改善患儿的血气分析指标,促进患儿康复,满足家属护理需求,满意度高。

Objective To evaluate the impact of the application of clustered nursing on the blood gas analysis and mechanical ventilation time of newborns with severe pneumonia,and to improve the quality of care to promote the prognosis of newborns with severe pneumonia.Methods The 92 neonates with severe pneumonia admitted to our hospital from February 2018 to July 2019 were selected for treatment in our hospital.On the basis of symptomatic treatment,the control group and the intervention group were randomized number table group nursing intervention,46 cases in each group.The neonates with severe pneumonia in the control group were treated with routine nursing at the same time,and the neonates with severe pneumonia in the intervention group were combined with clustered nursing intervention on the basis of the control group.The treatment status of the newborn with severe pneumonia between the control group and the intervention group was compared,including the improvement of blood gas analysis index level,mechanical ventilation time,cough disappearance time,wheeze disappearance time and family care satisfaction.Results The mechanical ventilation time,cough disappearance time and wheeze disappearance time of the intervention group were significantly shorter than those of the control group,P<0.05;the nursing satisfaction of the family members of the severe pneumonia newborns in the intervention group was significantly higher than that of the control group,P<0.05.The pH,oxygen saturation(SpO2),and arterial partial pressure of oxygen(PaO2)levels improved after intervention in the two groups,and the blood gas analysis indicators of the intervention group improved more significantly,P<0.05.Conclusion The symptomatic supportive treatment of neonates with severe pneumonia combined with clustered nursing intervention has a significant effect,which can further improve the blood gas analysis index of the child,promote the recovery of the child,and meet the nursing needs of family members with high satisfaction.
