闭合式痔切除术与内镜下硬化治疗术治疗Ⅱ~Ⅲ期内痔的临床效果比较

Comparison of clinical effects of closed hemorrhoidectomy and endoscopic sclerotherapy in the treatment of stageⅡ-Ⅲinternal hemorrhoids

目的比较闭合式痔切除术与内镜下硬化治疗术治疗Ⅱ~Ⅲ期内痔的临床效果。方法选择我院2019年2月至2019年6月收治的100例Ⅱ~Ⅲ期内痔患者为研究对象,根据治疗方式将其分为对照组(40例,闭合式痔切除术)和观察组(60例,内镜下硬化治疗术)。比较两组的临床疗效、手术指标、肛门功能及生活质量。结果观察组的治疗总有效率高于对照组(P<0.05)。观察组的手术时间、创面愈合时间、首次排便时间、住院时间、住院费用均优于对照组(P<0.05)。治疗后,两组的肛管静息压、直肠最大耐受量、肛管最大收缩压及Wexner、EQ-5D-5L评分均改善,且观察组优于对照组(P<0.05)。结论内镜下硬化治疗术治疗Ⅱ~Ⅲ期内痔效果显著,可改善患者的肛门功能,提高其生活质量,值得推广。

Objective To compare the clinical effects of closed hemorrhoidectomy and endoscopic sclerotherapy in the treatment of stageⅡ-Ⅲinternal hemorrhoids.Methods A total of 100 patients with stageⅡ-Ⅲinternal hemorrhoids admitted in our hospital from February 2019 to June 2019 were selected as the research objects and divided into control group(40 cases,closed hemorrhoidectomy)and observation group(60 cases,endoscopic sclerotherapy)according to the treatment methods.The clinical efficacy,surgical indicators,anal function and quality of life of the two groups were compared.Results The total effective rate of treatment of the observation group was higher than that of the control group(P<0.05).The operation time,wound healing time,first defecation time,hospital stay and hospitalization cost in the observation group were better than those in the control group(P<0.05).After treatment,the anal resting pressure,maximum rectal tolerance volume,maximum anal systolic pressure,Wexner and EQ-5D-5L scores improved in both groups,and those in the observation group were better than the control group(P<0.05).Conclusion Endoscopic sacrostherapy is effective in the treatment of stageⅡ-Ⅲinternal hemorrhoids,it can improve the anal function and quality of life of patients,which is worthy of promotion.
