多巴酚丁胺联合螺内酯治疗重症心力衰竭患者的临床疗效探讨

Clinical Efficacy of Dobutamine Combined with Spironolactone in the Treatmentof Severe Heart Failure

目的:观察多巴酚丁胺联合螺内酯治疗重症心力衰竭患者的临床疗效。方法:选取2019年11月~2020年9月某院收治的重症心力衰竭患者89例,随机分为对照组(44例)和研究组(45例),患者入院后均给予平衡酸碱度、电解质,吸氧,补液,洋地黄,β受体阻滞剂等常规治疗,之后对照组应用螺内酯治疗,研究组在此基础上应用多巴酚丁胺,两组患者均治疗28d后观察效果。比较两组不良反应发生率、心力衰竭复发率、心功能指标及治疗效果。结果:研究组不良反应发生率和疾病复发率均低于对照组(P<0.05);研究组治疗后BNP、LVESD、LVEDD低于对照组,LVEF高于对照组(P<0.05);研究组治疗效果高于对照组(P<0.05)。结论:螺内酯和多巴酚丁胺联合治疗重症心力衰竭患者效果较好,可以改善患者的肺功能,且安全性高,不良反应少,值得推广和应用。

Objective:To observe the clinical efficacy of dobutamine combined with spironolactone in the treatment of patients with severe heart failure.Methods:89 patients with severe heart failure in a hospital from November 2019 to September 2020 were selected and randomly divided into control group and study group,with 45 cases in study group and 44 cases in control group.After admission,all patients were given routine treatment such as balance of pH,electrolyte,oxygen inhalation,fluid infusion,digitalis,beta receptor blocker,then the control group was given spironolactone,on this basis,dobutamine was given intravenously in the study group.The effects of two groups of patients were observed after 28 days of treatment.The incidence of adverse reactions,recurrence rate of heart failure,cardiac function related indicators and treatment effects were compared between the two groups.Results:The incidence of adverse reactions and disease recurrence rate of the study group were lower than those of the control group(P<0.05);BNP,LVESD and LVEDD of the study group after treatment were lower than those of the control group,LVEF was higher than that of the control group(P<0.05);The treatment effect of the study group was higher than that of the control group(P<0.05).Conclusion:The effect of spironolactone and dobutamine in the treatment of patients with severe heart failure is good,which can improve the lung function of patients,has high safety and less adverse reactions,and is worthy of promotion and application.
