2016—2018年上海市闵行区初筛为糖尿病高危人群的35岁及以上常住居民糖尿病筛查结果 被引量:3

Analysis of initially diabetes screening results of high-risk group of permanent residents who were aged 35 and above in Minhang District of Shanghai from 2016 to 2018

摘要 目的了解上海市闵行区糖尿病高危人群糖尿病前期和糖尿病的检出率,分析影响高危人群血糖代谢异常的危险因素,为改善社区糖尿病防治工作提供依据。方法 2016—2018年在上海市闵行区13家社区卫生服务中心集中动员辖区内年龄≥35岁的常住居民参与糖尿病风险评估,填写《上海市社区居民糖尿病风险评估表》,评估为糖尿病高危人群的对象集中参与现场筛查,收集研究对象的基本信息、糖尿病危险因素、血糖检测结果等信息。采用频数、百分数、均数及标准差描述人口学基本特征,采用卡方检验比较不同特征高危人群血糖筛查结果;采用二分类Logistic回归模型分析影响高危人群发生糖代谢异常的影响因素,检验水准α=0.05。结果 2016—2018年在14 687名糖尿病高危人群中共检出糖尿病前期2 759人、糖尿病2 372人,检出率分别为18.8%和16.2%。随着年龄的增长,糖尿病前期和糖尿病检出率均增加(P<0.05),且男性、未婚/离婚/丧偶人群、大专以下学历人群中糖尿病前期和糖尿病检出率较高。Logistic回归分析结果显示:年龄≥40岁、糖调节受损史、一级亲属2型糖尿病家族史、高血压、血脂异常、动脉粥样硬化性心脑血管疾病患者、超重/肥胖是居民糖尿病前期和糖尿病检出阳性的影响因素(P<0.05),同时静坐生活方式也是糖尿病检出阳性的影响因素(P<0.05)。结论闵行区糖尿病高危人群中糖尿病前期和糖尿病检出率较高,应继续在高危人群特别是重点高危人群中开展糖尿病筛查工作,以实现疾病的早期发现和干预。 Objective To know the detection rate of prediabetes and diabetes in high-risk population of diabetes in Minhang district, and to analyze the risk factors affecting abnormal blood glucose metabolism in high-risk population, so that to provide a basis for improving the prevention and treatment of diabetes in the community. Methods From 2016 to 2018,about 13 community health service centers in Minhang district mobilized permanent community residents aged 35 and above to participate in diabetes risk assessment,“Shanghai Community Residents Diabetes Risk Assessment Form” were filled, and the residents assessed as high-risk population for diabetes were centrally screened on-site. The basic information of residents, diabetes risk factors, blood sugar test results and other informations of the research subjects were collected. Frequency, percentage, mean and standard deviation were used to describe basic demographic characteristics of the population,and chisquare test was used to compare the results of blood glucose screening of high-risk groups with different characteristics;Two-class logistic regression was used to analyze the influencing factors of abnormal glucose metabolism in high-risk groups,and the test level α=0.05. Results From 2016 to2018,a total of 2 759 people were detected with pre-diabetes and 2 372 people with diabetes in 14687 high-risk residents of diabetes, and the detection rates were 18.8% and 16.2%, respectively.With the increase of age,the detection rates of prediabetes and diabetes increased(P<0.05). The detection rates of prediabetes and diabetes were higher among men(P<0.05), unmarried/divorced/widowed people and people with college degree and below. Logistic regression analysis results showed: the age ≥40 years old, history of impaired glucose regulation, family history of type 2 diatetes mellitus in first-degree relatives,hypertension,dyslipidemia,atherosclerotic cardiovascular and cerebrovascular disease patients, overweight/Obesity was an influencing factor of prediabetes and diabetes positive detection(P<0.05), while sedentary lifestyle was also an influencing factor of diabetes positive detection(P<0.05). Conclusions The prediabetes and diabetes detection rates are relatively high among the high-risk residents of diabetes in Minhang district. Diabetes screening should be continued in high-risk people, especially the key high-risk people, so that to achieve early detection and intervention of the disease.
