摘要
目的:运用meta分析评价运动对糖尿病周围神经病变(DPN)患者的治疗作用。方法:在PubMed、web of science、中国知网等数据库搜索运动治疗DPN患者的随机对照试验,严格按照文献纳入与排除标准筛选文献,并进行文献质量评估,在review manage5.3软件上进行统计分析,选用加权均数差(WMD)作为效应量指标。结果:纳入10篇文献,共13个研究含685例患者。本meta分析结果显示运动可以增加DPN患者的腓总运动神经传导速度(PMCV)和腓总感觉神经传导速度(PSCV),降低DPN患者的糖尿病神经病变评分(MDNS)和多伦多临床神经病变评分(TCSS)评分。亚组分析发现运动干预总时长不宜太长、每周运动时间不超过3次并结合抗阻的运动可能对改善PMCV和PSCV具有更好的效果。结论:运动可以显著改善DPN患者的神经传导速度,且在对DPN患者进行运动治疗时应加强科学合理的保护与监控。
Objective:To evaluate the therapeutic effect of exercise on diabetic peripheral neuropathy(DPN)by using Meta-analysis.METHODS:PubMed,Web of Science,CNKI and other databases were searched the randomized controlled trials of exercise therapy for patients with DPN,the literatures were selected?according to the inclusion and exclusion criteria and the literatures quality was evaluated by the Cochrane evaluation tool.Statistical analysis was conducted on Review Manage5.3 software,and Weighted mean difference(WMD)was selected as effect size index.RESULTS:Ten literatures were included,including 13 studies with 685 patients.The results of this meta-analysis showed that exercise could increase the peroneal total motor nerve conduction velocity(PMCV)and peroneal total sensory nerve conduction velocity(PSCV)in patients with DPN,and decrease the diabetic neuropathy score(MDNS)and Toronto clinical neuropathy score(TCSS)in patients with DPN.The subgroup analysis of the study found that the time of exercise intervention should not be too long,the exercise frequency should not be more than 3 times per week and the aerobic exercise1 combined with resistance exercise may have a better effect on the improvement of PMCV and PSCV.Conclusion:Exercise can significantly improve the nerve conduction velocity of DPN patients,and scientific and reasonable protection and monitoring should be strengthened during exercise therapy for DPN patients.
