老年女性尿失禁患者的心理健康及社区护理干预效果分析

Mental health of elderly female patients with urinary incontinence and the effect of the community nursing intervention

目的:评价老年女性尿失禁患者的心理健康状况,分析社区护理干预的效果。方法:收集2020年2月到2021年2月上海市嘉定区江桥镇社区卫生服务中心收治的老年女性尿失禁患者22例作为观察组,并选择同期收治的其他疾病患者22例作为对照。对所有研究对象均利用90项症状自评量表(symptom check-list 90,SCL-90)进行心理健康评估。对观察组患者采用6个月“教-指-引-随”综合护理干预,比较患者护理前后的心理健康差异,并和对照组进行比较,同时利用老年尿失禁自我效能量表(geriatric self-efficacy scale for urinary incontinence,GSE-UI)评价自我效能情况。结果:观察组在社区综合护理后,SCL-90评分明显低于护理前(P<0.05),但依然高于对照组,差异无统计学意义(P>0.05)。护理后,观察组患者的控尿意识评分、自我控制漏尿信息评分、克服阻碍盆底肌锻炼的信心评分、盆底肌锻炼和对其治疗的信心评分都明显高于护理前(P<0.05)。结论:老年女性尿失禁患者都存在严重的心理健康问题,积极的社区综合护理可以改善患者心理健康问题,提高患者自我效能,对改进生活质量有重要作用,值得推荐。

Objective:To evaluate the mental health status of elderly female patients with urinary incontinence and to analyze the effect of the community nursing intervention.Methods:Twenty-two elderly female patients with urinary incontinence in Jiangqiao Community Health Service Center of Jiading District,Shanghai from February 2020 to February 2021 were collected as the observation group,and 22 patients with other diseases admitted during the same period were selected as the control group.All the study subjects used the symptom check-list 90(SCL-90)for mental health assessment.The observation group of patients adopted“teaching-pointing-citation-following”comprehensive nursing intervention for 6 months,and the mental health differences of patients before and after nursing were compared and compared with those of the control group.Meanwhile,geriatric self-efficacy scale for urinary incontinence(GSE-UI)was used to evaluate the self-efficacy.Results:The SCL-90 score of the observation group after community comprehensive nursing was significantly lower than that before nursing(P<0.05),and still higher than that of the control group,and the difference was not statistically significant(P>0.05).After nursing,the score of urinary control awareness,self-control urine leakage information score,confidence score of overcoming obstruction of pelvic floor muscle exercise,pelvic floor muscle exercise and confidence score of treatment of the observation group were significantly higher than before nursing(P<0.05).Conclusion:Elderly female patients with urinary incontinence all have serious mental health problems,active community comprehensive nursing can improve the mental health problems of patients,improve the self-efficacy of patients,and play an important role in improving the quality of life,which is worthy of recommendation.
