医联体背景下脑卒中患者延续护理服务管理模式评价

Evaluation of transitional care service management model for stroke patients under the background of Medical Cluster

目的:探讨医联体背景下脑卒中患者延续护理服务管理模式效果并提出相关对策建议。方法:选取医院80例脑卒中患者,采用简单随机抽样将研究对象分试验组(n=40)和对照组(n=40),对照组患者接受常规延续护理,试验组患者接受以医联体主导的延续护理服务。观察两组患者干预前、干预后3个月Barthel指数量表、Fugl-Meyer运动功能评分,出院后3个月的生命质量评分(QOL)、依从性评分及护理体验感情况。结果:干预前,两组患者Barthel指数评分、Fugl-Meyer运动功能和NIHSS评分比较差异无统计学意义(P>0.05);干预后3个月试验组患者在Barthel指数评分、Fugl-Meyer运动功能评分和护理工作体验感方面均高于对照组(P<0.05),QOL各项评分和总分均高于对照组(P<0.05),试验组在服药、锻炼、复诊和饮食方面的依从性分别是对照组的28.83倍、8.14倍、21.00倍和10.23倍。结论:医联体延续护理服务管理模式能为出院患者提供不间断的护理服务,可有效改善患者日常生活能力、肢体运动功能,提高患者治疗依从性,提升患者护理体验感。

Objective:To explore the effect of transitional care service management mode for stroke patients under the background of Medical Cluster and put forward relevant suggestions.Methods:A total of 80 stroke patients admitted to the hospital from July 2019 to December 2019 and June 2020 to September 2020 were selected.All patients were divided by simple random sampling method.Totally 40 patients were included in the control group,treated with transitional care.The other 40 patients were included in the observation group,treated with transitional care services led by the Medical Cluster.Barthel index scale,Fugl-Meyer motor function,quality of life(QOL),compliance score and nursing satisfaction were observed before and 3 months after discharge in two groups.Results:Before the intervention,the Barthel index score,Fugl-Meyer motor function score,and NIHSS score of the two groups of patients were not significantly different between the groups(P>0.05).Three months after the intervention,the Barthel index score,Fugl-Meyer motor function score and overall satisfaction rate of nursing satisfaction work in the experimental group were higher than those in the control group(P<0.05),and all QOL scores and total scores were higher than those in the control group(P<0.05).The compliance of the experimental group in taking medication,exercise,further consultation,and diet was 28.826 times,8.143 times,21.000 times,and 10.231 times of the control group,respectively.Conclusion:The model of transitional care service management of Medical Cluster can provide continuous care services for discharged patients,effectively improve their daily living ability and limb movement function,improve their treatment compliance,and improve their sense of nursing experience.
