电刺激及生物反馈配合凯格尔运动治疗轻中度盆腔脏器脱垂的效果 被引量:1

Effect of electrical stimulation and biofeedback combined with Kegel exercise in the treatment of mild to moderate pelvic organ prolapse

摘要 目的:探讨电刺激及生物反馈配合凯格尔运动治疗轻中度盆腔脏器脱垂的效果。方法:选取2019年5月—2020年5月接诊的100例轻中度盆腔脏器脱垂患者,按照随机数字表法分为对照组和观察组,每组50例。对照组接受电刺激及生物反馈治疗,观察组在对照组基础上接受家庭凯格尔运动训练。比较两组患者治疗前后盆底表面肌电检查结果、盆腔脏器下垂情况以及腹肌参与度。结果:观察组治疗后前静息阶段平均值、快肌阶段最大值、慢肌阶段平均值、后静息阶段平均值均高于对照组,快肌阶段上升时间和恢复时间均短于对照组,差异有统计学意义(P<0.05);两组治疗后前静息阶段变异性、慢肌阶段变异性以及后静息阶段变异性均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组治疗后盆腔器官脱垂定量分度法(POP-Q)分级均优于治疗前,且观察组优于对照组,差异有统计学意义(P<0.05);观察组治疗后腹肌参与度<10%所占比例高于对照组,差异有统计学意义(P<0.05)。结论:电刺激及生物反馈治疗配合凯格尔运动治疗轻中度盆腔脏器脱垂,效果良好,能够有效改善患者的盆底肌力,促进脏器脱垂恢复。 Objective:To investigate the effect of electrical stimulation and biofeedback combined with Kegel exercise in the treatment of mild to moderate pelvic organ prolapse.Methods:According to the random number table method,100 patients with mild to moderate pelvic organ prolapse in Pingxiang maternal and child health hospital from May 2019 to may 2020 were divided into two groups,50 cases in each group.The control group received electrical stimulation and biofeedback therapy,and the observation group received family Kegel exercise training on this basis.The results of electromyogram,pelvic organ prolapse and abdominal muscle participation were compared between the two groups before and after treatment.Results:After treatment,the average value of pre rest stage,maximum value of fast muscle stage,average value of slow muscle stage and average value of post rest stage of observation group were higher than those of control group,the rising time and recovery time of fast muscle stage were shorter than those of control group,the difference between groups was significant(P<0.05).After treatment,the variability of pre rest stage,slow muscle stage and post rest stage of the two groups were lower than those before treatment,with significant difference within the group(P<0.05).After treatment,the variability of pre rest stage,slow muscle stage and post rest stage in the observation group were lower than those in the control group after treatment,and the difference between the two groups was significant(P<0.05).After treatment,pelvic organ prolaps quantitation(POP-Q)scores of the two groups were better than those before treatment,and the difference was significant(P<0.05).The POP-Q grade of the observation group was better than that of the control group after treatment,and the difference was significant(P<0.05).After treatment,the proportion of abdominal muscle participation less than 10%in the observation group was higher than that in the control group,and the difference between the two groups was significant(P<0.05).Conclusion:Electrical stimulation and biofeedback therapy combined with Kegel exercise has a good therapeutic effect on mild to moderate pelvic organ prolapse,which can effectively improve the pelvic floor muscle strength of patients and promote the recovery of organ prolapse.
