螺旋CT水对比造影在胃癌检查中的应用

Application of spiral CT water contrast radiography in the examination of gastric cancer

目的探讨水对比造影在胃癌螺旋CT检查中的应用价值。方法选择2018年2月至2020年2月延安大学咸阳医院医学影像科检查的72例疑似胃癌患者为研究对象,根据随机数表法将疑似患者分为实验组和对照组各36例,对照组采用气钡双重造影检查,实验组采用螺旋CT水对比造影,以术后病理学检查结果作为诊断标准,比较两种检查方法对胃癌的准确率及敏感度、特异度、阳性预测值与阴性预测值,比较两组患者的黏膜病变、软组织肿块、胃壁增厚以及食管下端变窄的检出率。结果实验组对胃癌诊断的准确率、敏感度、特异度、阳性预测值以及阴性预测值分别为96.88%、96.88%、75.00%、96.88%、75.00%,明显高于对照组的76.92%、76.92%、60.00%、83.33%、50.00%,差异均有统计学意义(P<0.05);实验组对黏膜病变、软组织肿块、胃壁增厚以及食管下端变窄的检出率分别为88.89%、77.78%、58.33%、72.22%,明显高于对照组的69.44%、61.11%、44.44%、58.33%,差异均有统计学意义(P<0.05)。结论相较于气钡双重造影技术,螺旋CT水对比造影用于胃癌的检查效果更理想,可有效提高胃癌诊断效能,临床上可推广应用。

Objective To analyze the application of spiral CT water contrast radiography in the examination of gastric cancer.Methods From February 2018 to February 2020,72 patients with suspected gastric cancer in Xianyang Hospital of Yan'an University were selected and divided into an experimental group and a control group according to different examination methods,with 36 patients in each group.The control group was examined by gas barium double contrast examination,and the experimental group was examined by spiral CT water contrast radiography.The results of postoperative pathological examination were used as the diagnostic criteria.The accuracy,sensitivity,specificity,positive predictive value,and negative predictive value of gastric cancer of the two groups were compared and analyzed.And the detection rate of mucosal lesions,soft tissue masses,gastric wall thickening,and lower esophageal narrowing were compared.Results The accuracy,sensitivity,specificity,positive predictive value,and negative predictive value of the experimental group for diagnosis of gastric cancer were 96.88%,96.88%,75.00%,96.88%,and 75.00%,which were significantly higher than 76.92%,76.92%,60.00%,83.33%,and 50.00%in the control group(P<0.05).The detection rates of mucosal lesions,soft tissue masses,thickening of gastric wall,and narrowing of lower end of esophagus in the experimental group were 88.89%,77.78%,58.33%,72.22%,which were significantly higher than 69.44%,61.11%,44.44%,58.33%in the control group(P<0.05).Conclusion Compared with air barium double contrast technique,spiral CT water contrast radiography for gastric cancer examination has ideal effect,which can effectively improve the diagnostic efficiency of gastric cancer and should be further applied in clinic.
