盐酸米诺环素软膏联合牙龈下刮治、根面平整术治疗牙周炎并牙龈出血效果观察

Clinical Efficacy of Minocycline Hydrochloride Ointment Combined with Subgingival Curettage and Root Planing in Treatment of Periodontitis and Gingival Bleeding

目的探讨盐酸米诺环素软膏联合牙龈下刮治、根面平整术对牙周炎并牙龈出血的治疗效果。方法回顾性分析2018年2月—2019年10月收治的牙周炎并牙龈出血81例的临床资料,根据治疗方法分为观察组41例和对照组40例。对照组单纯采用牙龈下刮治、根面平整术治疗,观察组在对照组基础上给予盐酸米诺环素软膏治疗。比较2组临床效果、治疗前后牙周指数及探针深度,检测2组治疗前后白细胞介素-10(IL-10)、转化生长因子-β(TGF-β)、辅助性T细胞(Th17)、调节性T细胞(Treg)水平,统计2组不良反应发生率。结果观察组总有效率高于对照组(P<0.05)。观察组治疗后各项牙周指数及探针深度均低于对照组,IL-10、TGF-β高于对照组,Th17、Treg低于对照组(P<0.05,P<0.01)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论盐酸米诺环素软膏联合牙龈下刮治、根面平整术治疗牙周炎并牙龈出血效果好,能够改善牙周指标、血清学指标,安全性较高。

Objective To investigate clinical efficacy of Minocycline Hydrochloride Ointment combined with subgingival curettage and root planing in treatment of patients with periodontitis and gingival bleeding.Methods Clinical data of 81 patients with periodontitis and gingival bleeding admitted between February 2018 and October 2019 was retrospectively analyzed.The patients were divided into observation group(n=41)and control group(n=40)according to different therapeutic methods.Control group was treated with subgingival curettage and root planing,while observation group was supplemented with Minocycline Hydrochloride Ointment on the basis of treatment for control group.In two groups,clinical efficacy,periodontal index and probe depth before and after treatment were compared,and levels of interleukin-10(IL-10),transforming growth factor-β(TGF-β),helper T cells(Th17)and regulatory T cells(Treg)before and after treatment were detected,and incidence rate of adverse reactions were recorded.Results The total effective rate in observation group was significantly higher than that in control group(P<0.05).After treatment in observation group,periodontal index and probe depth were significantly lower;IL-10 and TGF-βlevels were significantly higher,while Th17 and Treg levels were significantly lower than those in control group(P<0.05,P<0.01).There was no significant difference in incidence rate of adverse reactions between two groups(P>0.05).Conclusion Minocycline Hydrochloride Ointment combined with subgingival curettage and root planing is effective in treatment of patients with periodontitis and gingival bleeding.It may improve periodontal indexes and serology indexes with good safety.
