分析2型糖尿病患者甲状腺超声异常改变与甲状腺功能变化 被引量:1

Analysis of Abnormal Changes of Thyroid Ultrasound and Changes of Thyroid Function in Patients with Type 2 Diabetes

摘要 目的探讨甲状腺超声检查在2型糖尿病患者临床诊疗中的应用价值,分析甲状腺超声异常改变与甲状腺功能变化情况。方法选取该院2019年6月—2021年1月期间收治的78例2型糖尿病患者以及同期接受检查的80名健康体检者作为研究对象,分别作为观察组和对照组,比较两组的甲状腺超声检查结果。结果观察组的甲状腺结节检出率(53.85%>15.00%)、甲状腺增大检出率(17.95%>3.75%)、甲状腺回声异常检出率(24.36%>11.54%)高于对照组,差异有统计学意义(χ^(2)=26.492、8.292、4.353,P<0.05),观察组患者的三碘甲状原氨酸(T3)水平[(1.26±0.33)nmol/L<(1.70±0.45)nmol/L]、甲状腺激素(T4)水平[(83.02±9.07)nmol/L<(89.14±11.25)nmol/L]、游离三碘甲状腺原氨酸(FT3)水平[(3.59±0.52)pmol/L<(4.40±0.67)pmol/L]、游离甲状腺激素(FT4)水平[(14.05±1.19)pmol/L<(15.34±1.94)pmol/L]低于对照组,差异有统计学意义(t=6.994、3.759、8.475、5.023,P<0.05)。在2型糖尿病患者中,HbA1c含量>9.0%的患者甲状腺结节、甲状腺增大、甲状腺回声异常的检出率比HbA1c含量<7.5%、7.5%~9.0%的患者更高,差异有统计学意义(P<0.05)。HbA1c含量>9.0%的患者T3水平、T4水平、FT3水平、FT4水平比HbA1c含量<7.5%、7.5%~9.0%的患者更低,差异有统计学意义(P<0.05)。结论2型糖尿病患者接受甲状腺超声检查的过程中,可以根据甲状腺超声异常改变,判断其甲状腺功能变化,反映出2型糖尿病对于甲状腺功能的影响,能够为疾病的临床诊疗提供有价值的参考。 Objective To explore the application value of thyroid ultrasound in the clinical diagnosis and treatment of patients with type 2 diabetes,and to analyze the abnormal changes of thyroid ultrasound and changes in thyroid function.Methods A total of 78 patients with type 2 diabetes who were admitted to the hospital from June 2019 to January 2021 and 80 healthy subjects who underwent examinations during the same period were selected as the research objects,as the observation group and the control group,respectively.The thyroid ultrasound examination results of the two groups were compared.Results The detection rate of thyroid nodules(53.85%>15.00%),thyroid enlargement(17.95%>3.75%)and abnormal thyroid echo(24.36%>11.54%)in the observation group were significantly higher than those in the control group,the difference was statistically significant(χ^(2)=26.492,8.292,4.353,P<0.05).The levels of triiodothyronine(T3)[(1.26±0.33)nmol/L<(1.70±0.45)nmol/L],thyroid hormone(T4)[(83.02±9.07)nmol/L<(89.14±11.25)nmol/L],free triiodothyronine(FT3)[(3.59±0.52)pmol/L<(4.40±0.67)pmol/L]and free thyroid hormone(FT4)[(14.05±1.19)pmol/L<(15.34±1.94)pmol/L]was lower than that of the control group,the difference was statistically significant(t=6.994,3.759,8.475,5.023,P<0.05).In patients with type 2 diabetes,the detection rate of thyroid nodules,thyroid enlargement and abnormal echo of thyroid in patients with HbA1c content>9%was higher than that in patients with HbA1c content<7.5%and 7.5%-9.0%,the difference was statistically significant(P<0.05).The levels of T3,T4,FT3 and FT4 in patients with HbA1c content>9.0%were lower than those in patients with HbA1c content<7.5%and 7.5%-9.0%,the difference was statistically significant(P<0.05).Conclusion During the process of thyroid ultrasound examination in patients with type 2 diabetes,the changes in thyroid function can be judged based on the abnormal changes of thyroid ultrasound,reflecting the impact of type 2 diabetes on thyroid function,it can provide a valuable reference for clinical diagnosis and treatment of the disease.
