在糖尿病早期肾损伤诊断中应用尿糖与尿微量白蛋白联合检验对于提高诊断准确率的作用

The Role of Combined Test of Urine Glucose and Urine Microalbumin in the Diagnosis of Early Renal Injury in Diabetes to Improve the Accuracy of Diagnosis

目的分析尿糖与尿微量白蛋白联合检验对糖尿病早期肾损伤诊断准确率的提升效果。方法选取80例该院2020年1月—2021年1月期间收治的糖尿病患者,根据有无早期肾损伤分为研究组(合并早期肾损伤)和对照组(单纯糖尿病),各40例。两组均予以尿糖、尿微量白蛋白检测,比较其检测结果,并分析两者单独检测与联合应用对早期肾损伤的诊断效果。结果研究组尿糖水平为(3.89±0.57)mmol/L,尿微量白蛋白水平为(31.75±4.39)mg/L,显著高于对照组的(2.12±0.46)mmol/L和(15.86±2.42)mg/L,差异有统计学意义(P<0.05)。两指标联合检验对糖尿病早期肾损伤的诊断灵敏度为95.00%,特异度为92.50%,准确度为93.75%,阳性预测值为92.68%,阴性预测值为94.87%,与单独尿糖或尿微量白蛋白检验结果比较,差异有统计学意义(P<0.05)。结论两指标联合检验能够显著提升临床对糖尿病早期肾损伤的诊断灵敏度和准确度,能够为临床治疗提供有价值的参考信息。

Objective To analyze the effect of combined test of urine glucose and urine microalbumin on the diagnosis accuracy of early diabetic kidney injury.Methods Selected 80 diabetic patients admitted to the hospital from January 2020 to January 2021,and divided them into a study group(combined with early kidney injury)and a control group(simple diabetes)according to whether there was early kidney injury,with 40 cases in each.Both groups were tested for urine glucose and urinary microalbumin,the results were compared,and the diagnostic effects of the two separate and combined applications on early renal injury were analyzed.Results The urine glucose level in the study group was(3.89±0.57)mmol/L,and the urine microalbumin level was(31.75±4.39)mg/L,which was significantly higher than the control group(2.12±0.46)mmol/L and(15.86±2.42)mg/L,the difference was statistically significant(P<0.05).The sensitivity of the two-indicator joint test for early renal injury in diabetes was 95.00%,the specificity was 92.50%,the accuracy was 93.75%,the positive predictive value was 92.68%,and the negative predictive value was 94.87%.Compared with the results of urine glucose or urine microalbumin test alone,the difference was statistically significant(P<0.05).Conclusion The combined test of the two indicators can significantly improve the sensitivity and accuracy of clinical diagnosis of early diabetic kidney injury,and can provide valuable reference information for clinical treatment.
