摘要
目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患儿血纤维蛋白原(FIB)、红细胞计数(RBC)相关指标变化及其临床意义。方法回顾性分析2018年1月~2020年5月医院收治的90例OSAHS患儿的资料,记为A组,另回顾性分析同期在该院体检的84例健康儿童的资料,记为B组。对比A组和B组血FIB、RBC、血红蛋白(HB)、平均红细胞容积(MCV)、平均红细胞血红蛋白量(MCH)和平均红细胞血红蛋白浓度(MCHC)以及呼吸暂停低通气指数(AHI)、氧减指数(ODI)、夜间平均血氧饱和度(MnSaO_(2))和夜间最低血氧饱和度(LOSaO_(2)),对比A组不同病情严重程度患儿血FIB、RBC相关指标,睡眠呼吸参数,采用Pearson相关性检验,分析A组血FIB、RBC相关指标与睡眠呼吸参数的相关性。结果A组血FIB、RBC、HB、MCV、MCH、MCHC水平和睡眠呼吸参数AHI、ODI均高于B组(P<0.05),MnSaO_(2)和LOSaO_(2)均低于B组(P<0.05);A组不同病情严重程度患儿血FIB、RBC、HB、MCV、MCH、MCHC水平对比差异均有统计学意义(P<0.05),与轻度组相比,中度组和重度组均升高(P<0.05),重度组患儿又高于中度组(P<0.05);A组不同病情严重程度患儿AHI、ODI、MnSaO_(2)和LOSaO_(2)对比差异均有统计学意义(P<0.05),与轻度组相比,中度组和重度组AHI、ODI均升高(P<0.05),重度组患儿又高于中度组(P<0.05),MnSaO_(2)、LOSaO_(2)均降低(P<0.05),重度组患儿又低于中度组(P<0.05);A组患儿血FIB、RBC、HB、MCV、MCH、MCHC水平分别与AHI、ODI呈正相关(P<0.05),分别与MnSaO_(2)、LOSaO_(2)呈负相关(P<0.05)。结论OSAHS患儿血FIB、RBC、HB、MCV、MCH、MCHC水平及AHI、ODI均高于健康者,MnSaO_(2)和LOSaO_(2)均低于健康者,且OSAHS患儿血FIB、RBC相关指标与睡眠呼吸参数均相关。
Objective To explore the changes and clinical significance of blood fibrinogen(FIB)and red blood cell count(RBC)related indexes in children with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods The data of 90 children with OSAHS admitted to the hospital from January 2018 to May 2020 were retrospectively analyzed and recorded as the group A.During the same period,the data of 84 healthy children who were examined in the hospital were retrospectively analyzed and recorded as the group B.The blood FIB,RBC,hemoglobin(HB),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH)and mean corpuscular hemoglobin concentration(MCHC),as well as apnea hypopnea index(AHI),oxygen desaturation index(ODI),nocturnal mean oxygen saturation(MnSaO_(2))and nocturnal lowest oxygen saturation(LOSaO_(2))were compared between the two groups,and the blood FIB,RBC related indexes and sleep breathing parameters of children with different disease severity in the group A were compared.It used Pearson correlation test to analyze the correlation between blood FIB,RBC related indicators and sleep breathing parameters in the group A.Results The blood FIB,RBC,HB,MCV,MCH,MCHC levels and sleep breathing parameters AHI,ODI of the group A were higher than those of the group B(P<0.05),and MnSaO_(2)and LOSaO_(2)were lower than those of the group B(P<0.05).The blood FIB,RBC,HB,MCV,MCH,MCHC levels of children with different disease severity in the group A were statistically significant(P<0.05).Compared with the mild group,the moderate group and the severe group were increased(P<0.05),and these indicators of children in the severe group were higher than those in the moderate group(P<0.05).The levels of AHI,ODI,MnSaO_(2)and LOSaO_(2)of children with different disease severity in the group A were statistically significant(P<0.05).Compared with the mild group,the AHI and ODI of the moderate group and the severe group were increased(P<0.05),and these indicators of children in the severe group were higher than those in the moderate group(P<0.05).MnSaO_(2)and LOSaO_(2)were decreased(P<0.05),and these indicators of children in the severe group were lower than those in the moderate group(P<0.05).The blood FIB,RBC,HB,MCV,MCH,MCHC levels of children in the group A were positively correlated with AHI and ODI,respectively(P<0.05),and they were negatively correlated with MnSaO_(2)and LOSaO_(2),respectively(P<0.05).Conclusion The blood FIB,RBC,HB,MCV,MCH,MCHC levels and AHI,ODI of children with OSAHS are higher than those of healthy persons,and MnSaO_(2)and LOSaO_(2)are lower than those of healthy persons,and blood FIB and RBC related indexes of children with OSAHS are related to sleep breathing parameters.
