加温湿化高流量鼻导管通气对新生儿呼吸窘迫综合征血气指标的影响

摘要 目的研究加温湿化高流量鼻导管通气治疗新生儿呼吸窘迫综合征的临床疗效及对其动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)水平的影响。方法回顾性分析2018年1月至2021年6月在广东医科大学顺德妇女儿童医院(佛山市顺德区妇幼保健院)出生的200例呼吸窘迫综合征新生儿的临床资料,按治疗方法不同将其分为A组与B组,各100例。两组患儿均给予常规肺表面活性物质治疗,A组患儿在常规治疗的基础上联合经鼻持续气道正压通气治疗,B组患儿在常规治疗的基础上联合加温湿化高流量鼻导管通气治疗。比较两组患儿的临床疗效,治疗4、24、48 h后PaO2、PaCO2水平,以及治疗期间并发症发生情况。结果治疗后B组患儿的临床治疗总有效率显著高于A组;与治疗4 h后比,治疗24、48 h后两组患儿的PaO2水平呈逐渐升高趋势,而PaCO2呈逐渐降低趋势;治疗期间B组患儿的并发症总发生率显著低于A组(均P<0.05);而两组患儿组间PaO2、PaCO2经比较,差异均无统计学意义(均P>0.05)。结论相较于经鼻持续气道正压通气,新生儿呼吸窘迫综合征采用加温湿化高流量鼻导管通气治疗,可更好地提高临床治疗疗效,改善血气指标,促进呼吸功能的恢复,且安全性较高。
