2015-2020年重庆市垫江县脑卒中死亡率及疾病负担趋势研究

Analysis on trends of stroke mortality and disease burden of Dianjiang Country,Chongqing,2015-2020

目的 研究2015-2020年重庆市垫江县脑卒中死亡率和疾病负担变化趋势,为制定脑卒中防控政策提供建议。方法 通过中国疾病预防控制系统死因登记报告信息系统,选取垫江县2015-2020年脑卒中[国际疾病分类(ICD)-10:I60~I64]死亡个案资料。采用SPSS22.0统计软件分析死亡率、标化死亡率、伤残调整生命年(DALY)、早逝导致寿命损失(YLL)、伤残导致寿命损失(YLD)、去死因期望寿命等指标。2015-2020年率的趋势变化采用年度变化百分比(APC)表示,对APC的检验采用t检验,检验水准α=0.05。结果 垫江县脑卒中死亡率和标化死亡率分别从2015年131.87/10万、100.05/10万上升到2020年的183.61/10万、126.27/10万,男性高于女性,变化趋势差异均有统计学意义(P<0.05)。死亡率和标化死亡率随年龄段上升而上升,85岁及以上年龄段达到最高。YLL率由2015年的25.39‰上升到2020年的32.12‰,YLD率由2015年的3.74‰上升到2020年的4.41‰,DALY率由2015年的29.13‰上升至2020年的36.54‰,YLL率、YLD率和DALY率变化趋势差异均有统计学意义(P<0.05),且总体上男性均大于女性。去掉脑卒中死亡后,期望寿命由2015年的80.13岁增加到2020年的80.43岁,寿命损失率由2.88%上升至3.13%,年均增幅为3.77%,变化趋势差异均无统计学意义(P>0.05)。结论 2015-2020年重庆市垫江县脑卒中疾病负担整体呈增长趋势,全县卒中防治仍面临巨大挑战,防治力度亟待进一步加强。

Objective To analyze the trends of the mortality and disease burden caused by stroke in Dianjiang County,Chongqing,from 2015 to 2020,and to provide a reference for formulating prevention and control policies.Methods The data of stroke[International Classification of Diseases(ICD)-10:I60-I64]in Dianjiang County,Chongqing,from 2015 to 2020 were selected from“National mortality registration and reporting system of Chinese CDC”.The mortality rate,standardized mortality rate,disability-adjusted life years(DALY),life loss due to early death(YLL),life loss due to disability(YLD),and cause eliminated life expectancy were calculated by SPSS 22.0.Annual percent change(APC)was adopted for the changing trend of rate from 2015 to 2020,and the t-test was carried out on APC with the test levelα=0.05.Results The mortality rate and standardized mortality rate of stroke in Dianjiang County,Chongqing increased from 131.87/105 and 100.05/105 in 2015 to 183.61/105 and 126.27/105 in 2020,respectively,with males higher than females,and the differences in trends were statistically significant(P<0.05).The mortality rate and standardized mortality rate of stroke increased with age,and reached the highest level over 85 years old.The YLL rate of stroke increased from 25.39‰in 2015 to 32.12‰in 2020,from 3.74‰in 2015 to 4.41‰in 2020 for YLD rate,and from 29.13‰in 2015 to 36.54‰in 2020 for DALY rate.The change trends of YLL rate,YLD rate and DALY rate were statistically significant(P<0.05),and males were greater than females in general.After the death of stroke was removed in Dianjiang County of Chongqing,life expectancy increased from 80.13 years in 2015 to 80.43 years in 2020,and the life loss rate increased from 2.88%to 3.13%,with an annual increase of 3.77%,with no statistically significant(P>0.05).Conclusion The overall burden of stroke in Dianjiang County of Chongqing is increasing from 2015 to 2020.The prevention and treatment of stroke is still facing great challenges,and the control policies or measures need to be further strengthened.
