酸碱平衡失常在脓毒症中的临床意义 被引量:2

Clinical significance of acid-base balance disorder in sepsis

摘要 目的研究动脉血酸碱平衡失常与脓毒症间的关系,为脓毒症患者的早期诊断和干预提供理论依据。方法严格按照2016年美国重症医学会与欧洲重症医学会联合发布的脓毒症3.0定义及诊断标准,选取2016年1月—2020年10月本院住院的脓毒症患者80例进行动脉血血气分析,并对酸碱平衡失常情况进行评价。结果脓毒症患者极易发生酸碱平衡失常,其中单纯性酸碱平衡失常占20%(16例);双重性酸碱平衡失常占65%(52例),并以代谢性酸中毒合并呼吸性碱中毒为主(42例,52.5%);无酸碱平衡失常占15%(12例);无三重性酸碱平衡失常。结论脓毒症与酸碱平衡失常密切相关,酸碱平衡监测可辅助脓毒症的诊断,对脓毒症的早期诊断和干预具有重要的临床意义。 Objective To study the relationship between abnormal acid-base balance of arterial blood and sepsis and to provide theoretical basis for early diagnosis and intervention of sepsis patients.Methods Strictly in accordance with the definition and diagnostic criteria of sepsis 3. 0 jointly issued by the American Society for Critical Care Medicine and the European Society for Critical Care Medicine in 2016, 80 patients with sepsis hospitalized in our hospital from January 2016 to October 2020 were selected for arterial blood gas analysis, and the acid-base balance disorders were evaluated and statistically analyzed.Results Sepsis patients were prone to acid-base balance disorders, and simple acid-base balance disorders accounted for20%(16 cases). The dual acid-base balance disorder accounted for 65%(52 cases), and metabolic acidosis combined with respiratory alkalosis was the main cause(42 cases, 52. 5%). No abnormal acid-base balance accounted for 15%(12 cases). No triple acid-base balance disorder was observed.Conclusion Sepsis is closely related to the disorder of acid-base balance, and the monitoring of acid-base balance can be used to assist the diagnosis of sepsis, which has important clinical significance for the early diagnosis and intervention of sepsis.
