高萍治疗免疫性血小板减少症临床用药规律研究

Study on Clinical Administration Rules of GAO Ping in Treating Immune Thrombocytopenia

目的:基于数据挖掘技术对高萍教授治疗免疫性血小板减少症(ITP)的用药规律进行研究。方法:收集高萍教授治疗的ITP患者97例病案;筛选处方504个,其中血热妄行证74个,阴虚火旺证330个,脾虚失摄证50个,脾肾阳虚证50个。运用MedcaseV3.2仓公诊籍国医脉案数据记录挖掘分析系统,整理相关资料,对频数、药物之间的相关度、聚类进行分析。结果:阴虚火旺证的核心药对为女贞子、旱莲草,核心方剂为女贞子、旱莲草、茜草、黄芩、生地黄、甘草。血热妄行证的核心药对为生地黄、玄参,核心方剂为水牛角、生地黄、牡丹皮、玄参、赤芍、鸡血藤、仙鹤草。脾虚失摄证的核心药对为党参、白术,核心方剂为党参、白术、黄芪、甘草、当归、鸡血藤、仙鹤草。脾肾阳虚证的核心药对为菟丝子、淫羊藿,核心方剂为淫羊藿、补骨脂、女贞子、五味子、熟地黄、仙鹤草。结论:高萍教授治疗ITP将其分为血热妄行、阴虚火旺、脾虚失摄、脾肾阳虚4个证型,治疗时始终注重顾护脾胃,凉血、止血、化瘀贯穿始终,兼顾补肾、养血。

Objective:To study the administration rules of Prof.GAO Ping in the treatment of immune thrombocytopenia(ITP)based on data mining technology.Methods:A total of 504 prescriptions of 97 patients with ITP treated by Prof.GAO Ping were collected.The relevant data was collated and the frequency,and correlation and clustering of medicine were analyzed by Medcase V3.2 Canggong public medical record data recording and mining system.Results:The core couplet medicinal for syndrome of yin deficiency with effulgent fire consists of Fructus Ligustri Lucidi and Herba Ecliptae,and the core prescription includes Fructus Ligustri Lucidi,Herba Ecliptae,Radix et Rhizoma Rubiae,Radix Scutellariae,Radix Rehmanniae,and Radix et Rhizoma Glycyrrhizae.The core couplet medicinal for syndrome of frenetic movement of blood due to heat consists of Radix Rehmanniae and Radix Scrophulariae,and the core prescription includes Buball Cornu,Radix Rehmanniae,Moutan Cortex,Radix Scrophulariae,Paeoniae Radix Rubra,Spatholobi Caulis and Herba Agrimoniae.The core couplet medicinal for syndrome of spleen deficiency with failure consists of Radix Codonopsis and Rhizoma Atractylodis Macrocephalae,and the core prescription includes Radix Codonopsis,Rhizoma Atractylodis Macrocephalae,Radix Astragali,Radix et Rhizoma Glycyrrhizae,Radix Angelicae Sinensis,Spatholobi Caulis and Herba Agrimoniae.The core couplet medicinal for syndrome of spleen-kidney yang deficiency consists of Semen Cuscutae and Herba Epimedii,and the core prescription includes Herba Epimedii,Fructus Psoraleae,Fructus Ligustri Lucidi,Fructus Schisandrae Chinensis,Radix Rehmanniae Praeparata and Herba Agrimoniae.Conclusion:Treating ITP,Prof.GAO Ping divides the disease into four syndromes which are yin deficiency with effulgent fire,yin deficiency with effulgent fire,frenetic movement of blood due to heat and spleen-kidney yang deficiency,and always pays attention to protecting the spleen and stomach,cooling the blood,hemostasis and resolving stasis,and combines with tonifying the kidney and nourishing the blood.
