益气补肺膏联合吸入性糖皮质激素治疗儿童哮喘缓解期的临床观察

Clinical Observation of Yiqi Bufei Soft Extract Combined with Inhaled Corticosteroid in Treating Asthmatic Children in Remission Period

【目的】观察益气补肺膏联合吸入性糖皮质激素(ICS)治疗儿童哮喘缓解期的临床疗效,为临床哮喘缓解期中西医结合治疗提供科学依据。【方法】将69例哮喘缓解期患儿随机分为丙酸氟替卡松喷雾剂组(ICS组)26例、益气补肺膏组(膏方组)22例和ICS+膏方组21例。ICS组给予丙酸氟替卡松喷雾剂治疗,膏方组给予益气补肺膏口服治疗,ICS+膏方组给予丙酸氟替卡松喷雾剂联合益气补肺膏治疗。3个月为1个疗程,连续治疗6个月。观察3组患儿治疗前后的哮喘发作次数及中医证候积分的变化情况。【结果】(1)哮喘发作次数方面:治疗3个月后,3组患儿的哮喘发作次数均较治疗前减少(P<0.05);治疗6个月后,3组患儿的哮喘发作次数又较治疗3个月后减少(P<0.05)。组间比较,ICS组和ICS+膏方组患儿在治疗3个月和6个月后的哮喘发作次数均少于膏方组,差异均有统计学意义(P<0.05);ICS+膏方组患儿的哮喘发作次数有少于ICS组的趋势,但差异无统计学意义(P>0.05)。(2)中医证候积分方面:治疗6个月后,3组患儿的中医证候积分均较治疗前降低(P<0.01),且膏方组和ICS+膏方组的中医证候积分均明显低于ICS组,差异均有统计学意义(P<0.01);而膏方组与ICS+膏方组比较,差异无统计学意义(P>0.05)。【结论】对于哮喘缓解期患儿,ICS+膏方组的疗效优于ICS组和膏方组,既可以有效减少哮喘发作次数,又可以明显改善中医临床症状。

Objective To observe the clinical efficacy of Yiqi Bufei Soft Extract combined with inhaled corticosteroid in treating asthmatic children in remission period,so as to provide scientific basis for the clinical treatment of asthma in the remission period with integrated Chinese and western medicine. Methods Sixty-nine asthmatic children in the remission period were randomly divided into the inhaled corticosteroid spray group(ICS group, 26 cases), Yiqi Bufei Soft Extract group(soft extract group, 22 cases)and ICS+ soft extract group(21 cases). The ICS group was given inhaled corticosteroid spray of fluticasone propionate spray,the soft extract group was given oral use of Yiqi Bufei Soft Extract,and the ICS+ soft extract group was given inhaled fluticasone propionate spray combined with oral use of Yiqi Bufei Soft Extract. Three months constituted a course of treatment and the treatment lasted for 6 consecutive months. The changes of asthma attack frequency and traditional Chinese medicine(TCM)syndrome scores in the three groups were observed before and after treatment. Results(1)The frequency of asthma attacks:after 3 months of treatment,the frequency of asthma attacks in the 3 groups was reduced compared with that before treatment(P<0.05),and the frequency of asthma attacks in the 3 groups after6 months of treatment was decreased compared with that after 3 months of treatment(P<0.05). The intergroup comparison showed that the frequency of asthma attacks in ICS group and ICS+ soft extract group after 3 and 6 months of treatment was less than that in the soft extract group,and the difference was statistically significant(P<0.05).The frequency of asthma attacks in ICS+ soft extract group showed a decreasing trend in comparison with that in ICS group,but the difference was not statistically significant(P>0.05).(2)TCM syndrome scores:after 6 months of treatment,the TCM syndrome scores of the 3 groups were decreased in comparison with those before treatment(P<0.01),and the scores of the soft extract group and the ICS+ soft extract group were decreased in comparison with those in the ICS group,the difference being statistically significant(P<0.01). There was no significant difference between the soft extract group and ICS+ soft extract group(P>0.05). Conclusion For the treatment of children with asthma in the remission period,the efficacy of ICS+ soft extract is superior to that of the ICS or the soft extract alone,which can not only effectively reduce the frequency of asthma attack,but also significantly relieve the TCM symptoms of the patients.
