摘要
目的探讨氢氧化钙根管消毒剂对于牙髓炎、根尖周炎患者根管治疗的效果。方法选取2019年10月—2020年10月该院诊治的80例(120颗牙)牙髓炎、根尖周炎患者,按照随机数表法分为两组,对照组40例(60颗牙),观察组40例(60颗牙)。两组均给予根管治疗,对照组给予朗力牌(Ⅱ型)抑菌液Fc治疗,观察组给予氢氧化钙根管消毒剂治疗。比较两组牙周指数、根管疼痛程度变化、临床疗效、血清炎症因子水平。结果观察组总有效率95.00%高于对照组的75.00%,差异有统计学意义(χ^(2)=6.274,P<0.05)。治疗后,观察组牙周袋探诊深度(PD)、牙龈指数(GI)及菌斑指数(PLI)牙周指数分别为(2.08±0.31)mm、(0.72±0.07)分、(0.86±0.22)分均低于对照组(2.89±0.35)mm、(0.94±0.14)分、(1.17±0.26)分,差异有统计学意义(P<0.05);观察组根管疼痛程度要低于对照组,差异有统计学意义(Z=2.134,P<0.05);观察组炎性因子-6(IL-6)、炎性因子-8(IL-8)、炎性因子(IL-1β)及肿瘤坏死因子-α(TNF-α)等血清炎症因子水平分别为(2.53±0.53)、(418.25±21.41)、(612.55±20.66)、(57.23±2.44)ng/L均低于对照组(3.16±0.46)、(462.36±20.26)、(687.61±20.78)、(62.88±2.64)ng/L,差异有统计学意义(P<0.05)。结论氢氧化钙根管消毒剂对于牙髓炎、根尖周炎患者,在根管治疗中,可提高总有效率,改善炎症因子水平,降低PD、GI等牙周指数,效果理想。
Objective To investigate the effect of calcium hydroxide root canal disinfectant on root canal treatment in patients with pulpitis and periapical periodontitis. Methods From October 2019 to October 2020, 80 patients(120 teeth) with pulpitis and periapical periodontitis diagnosed and treated in the hospital were selected. According to the random number table method, they were divided into two groups, 40 cases in the control group(60 teeth) and 40 cases in the observation group(60 teeth). Both groups were given root canal treatment, the control group was given Langli brand(type Ⅱ) antibacterial solution Fc treatment, and the observation group was given calcium hydroxide root canal disinfectant treatment. The periodontal index, changes in the degree of root canal pain, clinical efficacy, and serum inflammatory factor levels were compared between the two groups. Results The total effective rate of 95.00% in the observation group was higher than 75.00% in the control group, and the difference was statistically significant(χ^(2)=6.274, P<0.05). After treatment, the observation group’s periodontal pocket probing depth(PD), gingival index(GI) and plaque index(PLI) and other periodontal indexes(2.08 ±0.31)mm,(0.72 ±0.07)points,(0.86 ±0.22)points, all lower than the control group(2.89 ±0.35)mm,(0.94 ±0.14)points,(1.17 ±0.26)points, the difference was statistically significant(P <0.05). The degree of root canal pain in the observation group was lower than that in the control group, and the difference was statistically significant(Z =2.134, P <0.05). Serum levels of inflammatory factors such as inflammatory factor-6(IL-6), inflammatory factor-8(IL-8), inflammatory factor(IL-1β) and tumor necrosis factor-α(TNF-α) in the observation group(2.53 ±0.53)ng/L,(418.25±21.41)ng/L,(612.55±20.66)ng/L,(57.23±2.44)ng/L, lower than the control group(3.16 ±0.46)ng/L,(462.36 ±20.26)ng/L,(687.61 ±20.78)ng/L,(62.88 ±2.64)ng/L, the difference was statistically significant(P <0.05). Conclusion Calcium hydroxide root canal disinfectant is effective for patients with pulpitis and periapical periodontitis. In root canal treatment, it can increase the total effective rate, improve the level of inflammatory factors, and reduce periodontal indexes such as PD and GI, with ideal results.
