The Role of Adverse Event Monitoring in the Safety Management of Ventilator Clinical Use

目的分析不良事件监测在呼吸机临床使用安全管理中的作用。方法以2016年1月—2021年7月实施不良事件监测,依据安全管理的具体实施方法,应用相应的监测手法和工具,对48台呼吸机的具体情况进行有效归类,同时制订干预对策。结果设备自身出现故障的主要因素为:设备应用年限时间较久,设备的部件出现老化的状态,患者增多导致应用频率增加,设备保养没有及时跟进;应用品管圈降低连续性血液净化设备可控报警次数的方法科学有效可行。参数设置不当问题包括:呼吸上下限报警装置、窒息报警、触发灵敏度设置、呼吸末正压PEEP设置、吸气呼气流量设置。结论呼吸机临床使用安全管理中,不良事件监测的效果明显。

Objective To analyze the role of adverse event monitoring in the safety management of ventilator clinical use.Methods From January 2016 to July 2021,the implementation of adverse event monitoring,according to the specific implementation methods of safety management,application of corresponding monitoring methods and tools.Effectively classify the specific conditions of the 48 ventilators,and formulate intervention countermeasures at the same time.Results The main factors for the failure of the equipment itself were:the equipment had been used for a long time,and the components of the equipment appear to be aging.The increase in the number of patients had led to an increase in the frequency of applications,and equipment maintenance had not been followed up in time.The method of applying quality control circle to reduce the number of controllable alarms of continuous blood purification equipment was scientific,effective and feasible.The problems of improper parameter setting included:upper and lower respiratory limit alarm device,apnea alarm,trigger sensitivity setting,positive end-expiratory pressure PEEP setting,inspiratory expiratory flow setting.Conclusion In the safety management of the clinical use of ventilators,the effect of adverse event monitoring is obvious.
