参附注射液对脓毒症大鼠肺损伤的保护作用 被引量:7

摘要 目的观察参附注射液对脓毒症大鼠肺保护作用并探讨其可能的作用机制。方法30只健康无特定病原体(SPF)级SD大鼠随机分成空白对照组、模型组和参附组3组,每组10只。空白对照组尾静脉注射0.9%氯化钠注射液10 m L/kg加0.9%氯化钠注射液10 mL/kg;模型组尾静脉注射0.1%脂多糖(LPS)溶液10 mL/kg加0.9%氯化钠注射液10 m L/kg;参附组尾静脉注射0.1%LPS溶液10 m L/kg加参附注射液10 m L/kg。6 h后检测血气分析,记录肺湿/干重比(W/D),观察肺组织病理检测并记录肺组织损伤评分,酶联免疫吸附试验(ELISA)检测血清中肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-1(MMP-1)、Syndecan-1、硫酸乙酰肝素(HS)、硫酸软骨素(CS)水平。结果与空白对照组相比,模型组与参附组血pH低、乳酸(Lac)高、氧分压(PaO;)低、肺W/D比高,肺组织出现水肿伴炎细胞浸润、肺组织损伤评分升高,血清TNF-α、MMP-1、Syndecan-1、HS、CS水平高(P<0.05);与模型组相比,参附组血pH升高、Lac降低、PaO;明显改善、肺W/D比明显降低、镜下观察发现肺组织水肿及炎细胞浸润改善、肺组织损伤评分下降,血清TNF-α、MMP-1、Syndecan-1、HS、CS水平显著下降(P<0.05)。结论参附注射液具有一定的改善脓毒症肺损伤的作用。其机制可能是通过降低炎症水平,抑制MMP-1的过量表达,减少内皮多糖包被的降解,从而降低肺毛细血管的通透性、改善氧合,保护肺组织并减轻肺部及全身的炎症反应。 Objective:To observe the protective effect of Shenfu Injection on the lung of septic rats and explore its possible mechanism.Methods:Thirty healthy specific pathogen-free(SPF)SD rats were randomly divided into three groups:blank control group,model group and Shenfu group,10 rats in each group.The blank control group was given tail vein injection of 0.9%sodium chloride injection 10 mL/kg plus 0.9%sodium chloride injection 10 mL/kg;the model group was given tail vein injection of 0.1%lipopolysaccharide(LPS)solution 10 mL/kg plus 0.9%sodium chloride injection 10 m L/kg;the Shenfu group was injected with 0.1%LPS solution 10 mL/kg and Shenfu Injection 10 mL/kg through tail vein.After 6 hours,blood gas analysis was detected,lung wet/dry(W/D)weight ratio was recorded,lung tissue pathology was observed and lung tissue injury score was recorded,and serum tumor necrosis factor-α(TNF-α),matrix metalloproteinase-1(MMP-1),Syndecan-1,heparan sulfate(HS),chondroitin sulfate(CS)levels were detected by enzyme-linked immunosorbent assay(ELISA).Results:Compared with the blank control group,the model group and the Shenfu group had lower blood pH,higher lactate(Lac),lower PaO;,higher lung W/D ratio,improvement of lung tissue edema with inflammatory cell infiltration,and increased lung tissue injury score.Serum levels of TNF-α,MMP-1,Syndecan-1,HS,and CS were higher(P<0.05);compared with the model group,the Shenfu group had higher blood pH,lower Lac,significantly improved PaO;,and lung W/D ratio;the pulmonary tissue edema and inflammatory cell infiltration were improved,the lung tissue injury score was decreased,and the serum levels of TNF-α,MMP-1,Syndecan-1,HS,and CS were significantly decreased(P<0.05).Conclusion:Shenfu Injection has a certain effect on improving septic lung injury.The mechanism may be by reducing the level of inflammation,inhibiting the overexpression of MMP-1,reducing the degradation of endothelial polysaccharide coating,thereby reducing the permeability of pulmonary capillaries,improving oxygenation,protecting lung tissue and reducing inflammation reactions in the lungs and the whole body.
