摘要
目的检测循环血外泌体miR-27a、miR-27b表达与单纯性肥胖儿童胰岛素抵抗、血脂代谢和生长发育的关系。方法共纳入了120例年龄在6~15岁之间在我院儿保科体检的儿童,包括60例诊断为单纯性肥胖者作为肥胖组和60例非肥胖儿童作为正常对照组。进行人体测量(包括身高、体重、腰围等),采集血样,提取血清外泌体,采用实时荧光定量PCR法检测外泌体miR-27a和miR-27b相对表达量,另常规检测空腹血糖和胰岛素、血脂、血清瘦素、脂联素(ADP)、生长激素(GH)水平。结果与正常对照组相比,肥胖组包括男性亚组和女性亚组的儿童血清外泌体miR-27a、miR-27b相对表达量均升高(P<0.05)。另外,男性亚组和女性亚组儿童血清外泌体miR-27a、miR-27b相对表达量比较差异无统计学意义(P>0.05)。经Pearson相关性分析,肥胖组儿童外泌体miR-27a/b相对表达量与HOMA-IR(r=0.630、0.676,P<0.001)、TC(r=0.553、0.528,P<0.001)、TG(r=0.496、0.592,P<0.001)、瘦素(r=0.740、0.724,P<0.001)水平呈正相关性,同时与HDL-C(r=-0.379、-0.482,P<0.001)、ADP(r=-0.534、-0.516,P<0.001)、GH(r=-0.548、-0.589,P<0.001)呈负相关性。结论miR-27a/b相对表达量升高可能是导致单纯性肥胖儿童胰岛素抵抗、血脂代谢紊乱以及生长激素分泌失衡的重要原因。
Objective To investigate the relationship between the expression of mirR-27a/b and insulin resistance,blood lipid metabolism and growth and development in children with simple obesity.Methods A total of 120 children(6-15 years old)were included into the study,including 60 children diagnosed as simple obesity and 60 non-obese children as the normal control group.The subjects’body characteristics were measured(including height,weight,and waist circumference,etc.).The blood samples were collected,serum exosomes were extracted,and the relative expression of exosomal miR-27a and miR-27b were detected by real-time quantitative PCR.The fasting blood glucose and insulin,blood lipids,serum leptin,adiponectin(ADP),and growth hormone(GH)levels were measured routinely among all subjects.Results Compared with the normal control group,the relative expression of exosomal miR-27a and miR-27b in serum of obese children including male and female groups were increased(P<0.05),while there was no significant difference in the relative expression of miR-27a and miR-27b between male and female children(P>0.05).Pearson correlation analysis showed that the relative expressions of miR-27a and miR-27b were positively correlated with HOMA-IR(r=0.630,0.676,P<0.001),TC(r=0.553,0.528,P<0.001),TG(r=0.496,0.592,P<0.001),Leptin(r=0.740,0.724,P<0.001),while they were also negatively correlated with HDL-C(r=-0.379,-0.482,P<0.001),ADP(r=-0.534,-0.516,P<0.001)and GH(r=-0.548,-0.589,P<0.001).Conclusion The increased exosomal miR-27a/b levels may be an important cause of insulin resistance,dyslipidemia and imbalance of growth hormone secretion in children with simple obesity.
