养心方对缺血性脑卒中后睡眠障碍患者睡眠质量及血清5-HT、BDNF水平的影响

Effect of Yangxin Formula on Sleep Quality and Serum Levels of 5-HT and BDNF in Patients with Sleep Disorders after Ischemic Stroke

目的:探讨养心方联合黛力新对缺血性脑卒中后睡眠障碍患者睡眠质量及血清5-HT、BDNF水平的影响。方法:选择符合纳入标准的缺血性脑卒中后睡眠障碍患者78例,采用随机数字表随机分为对照组和观察组,每组39例。两组患者均给予常规对症治疗,对照组口服氟哌噻吨美利曲辛片,观察组在对照组基础上口服养心方,两组患者均连续治疗6周。对比两组患者治疗前和治疗后匹兹堡睡眠质量指数(PSQI)、临床神经功能缺损评分(CSS)、血清脑源性神经营养因子(BDNF)和5-羟色胺(5-HT)水平变化情况。结果:治疗后两组患者PSQI评分均降低(P<0.05),且观察组明显低于对照组(P<0.05);治疗后观察组治疗总有效率94.87%(37/39),明显高于对照组79.49%(31/39)(P<0.05);治疗后两组患者CSS评分均降低(P<0.05),且观察组明显低于对照组(P<0.05);治疗后两组患者血清5-HT、BDNF含量均升高(P<0.05),且观察组明显高于对照组(P<0.05)。结论:养心方联合黛力新对缺血性脑卒中后睡眠障碍患者疗效显著,可有效改善患者神经功能和睡眠质量,其作用机制可能与升高血清5-HT、BDNF含量有关。

Objective:To explore the effects of Yangxin Formula combined with Deanxit on sleep quality and serum levels of 5-HT and BDNF in patients with sleep disorders after ischemic stroke.Methods:78 patients,who met the inclusion criteria of sleep disorders after ischemic stroke,were randomly divided into the control group and the observation group,with 39 patients in each group.Both groups were given conventional symptomatic treatment.The control group was treated with Flupentixol and Melitroxine Tablet;on which basis,the observation group was also treated with Yangxin Formula.Both groups were treated for 6 consecutive weeks.The levels of PSQI,CSS,BDNF and 5-HT were compared between before and after the treatment in the two groups.The total effective rate was also compared between the two groups.Results:The PSQI score was decreased after the treatment in the two groups(P<0.05),of which the decrease in the observation group was more significant than that in the control group(P<0.05).The CSS score was reduced after the treatment in the two groups(P<0.05),of which the reduction was more significant in the observation group(P<0.05).The serum levels of 5-HT and BDNF were increased after the treatment in the two groups(P<0.05),of which the increases were more significant in the observation group(P<0.05).The total effective rate was 94.87%(37/39)in the observation group,which was significantly higher than 79.49%(31/39)in the control group(P<0.05).Conclusion:Yangxin Formula combined with Deanxit has a significant effect on patients with sleep disorders after ischemic stroke,and it can effectively improve the neurological function and sleep quality of the patients.Its action mechanism may be related to the increase of serum levels of 5-HT and BDNF.
