大蒜素联合头孢哌酮/舒巴坦对鲍曼不动杆菌体外作用效果分析 被引量:1

摘要 目的探讨大蒜素联合头孢哌酮/舒巴坦对鲍曼不动杆菌(AB)临床分离株的体外抗菌作用.方法采用棋盘法设计,微量肉汤稀释法倍比稀释2组抗菌药物组成不同浓度组合,分别测试30株头孢哌酮/舒巴坦敏感AB和30株头孢哌酮/舒巴坦耐药AB的最低抑菌浓度(MIC),并计算FIC指数判定联合作用的效果.结果当单独使用两种药物时,MIC值为大蒜素312.5~1,250 mg/L、头孢哌酮/舒巴坦8~512 mg/L.大蒜素与头孢哌酮舒巴坦联合使用后,耐药组中大蒜素和头孢哌酮/舒巴坦的MIC均有明显下降,差异有统计学意义(P<0.05);敏感组中头孢哌酮/舒巴坦单用与联用MIC比较,差异无统计学意义(P>0.05).结论大蒜素与头孢哌酮/舒巴坦联合使用后,对头孢哌酮/舒巴坦耐药的AB抗菌作用主要表现为协同或相加效应,大蒜素可提高头孢哌酮/舒巴坦AB的抑菌活性;对头孢哌酮/舒巴坦敏感的AB抗菌作用大部分表现为无关效应,大蒜素对提高头孢哌酮/舒巴坦AB的抑菌活性无明显作用. Objective To investigate the antibacterial effect of allicin combined with cefoperazone/sulbactam on Acinetobacter baumannii(AB)isolated from ICU patients in vitro.Methods Using the chessboard design,the microbroth dilution method was used to dilute two groups of antibiotics in different concentrations.The minimum inhibitory concentration(MIC)of 30 strains of cefoperazone/sulbactam sensitive AB and 30 strains of cefoperazone/sulbactam resistant AB were respectively tested,and the effect of the combined action was detennined by calculating FIC index.Results When the two dmgs were used alone,the MIC values ranged from 312.5 to 1,250 mg/L for allicin and 8~512 nig/L for cefoperazone/sulbactam.After allicin combined with cefoperazone and sulbactam,the MIC of allicin and cefoperazone/sulbactam in drug resistant group were significantly decreased,with P values of 0.000,which was statistically significant(P<0.05).MIC P value of cefoperazone/sulbactam alone was 0.099 compared with that of combined use in the sensitive group,showing no statistical significancc(P>0.05).Conclusion When combined with cefoperazone/sulbactam,allicin showes synergistic or additive effects on the antibacterial activity of cefoperazone/sulbactam resistant Acinetobacter baumannii,and allicin could significantly improve the antibacterial activity of cefoperazone/sulbactam resistant Acinetobacter baumannii.The antimicrobial activity of cefoperazone/sulbactam sensitive Acinetobacter baumannii is mostly independent,and allic in has no significant effect on the antibacterial activity of cefbperazone/sulbactam sensitive Acinetobacter baumannii.
