摘要
目的探讨分形维数(FD)在评价腰椎旁肌退变中的可行性及探讨其与下腰痛的相关性。方法搜集216例成人的腰椎MRI扫描结果及相关临床资料。采用Kjaer分级方法对腰椎旁肌的脂肪浸润程度进行分级。基于ImageJ软件计算各层面腰多裂肌FD。统计分析腰椎旁肌FD与Kjaer分级及下腰痛等因素的相关性。结果三个Kjaer等级组的平均FD:Kjaer 0:1.19±0.04;Kjaer 1:1.33±0.06;Kjaer 2:1.58±0.11,不同Kjaer分级组间FD均具有显著统计学差异(P<0.01)。回归分析发现腰椎旁肌FD与下腰痛具有显著相关性(P<0.01)。结论 FD方法能够有效评估腰椎旁肌退变过程,有望成为腰椎旁肌退变客观定量的检测工具。
Objective To investigate the feasibility of fractal dimension(FD) in the evaluation of lumbar paraspinal muscle degeneration and its correlation with lower back pain. Methods The MRI data of 216 adults and the related clinical data were collected. The fatty infiltration of lumbar paraspinal muscle was classified by Kjaer classification.The fractal dimension of lumbar paraspinal muscle was calculated based on ImageJ software. The correlation between the fractal dimension of paralumbar muscle and Kjaer grade and lower back pain were analyzed. Results The average fractal dimension of the three Kjaer groups was as follows: Kjaer 0:1.19±0.04;Kjaer 1:1.33±0.06;Kjaer 2:1.58±0.11.There were significant differences in fractal dimension between different Kjaer classification groups(P< 0.01). Regression analysis showed that the fractal dimension of paraspinal muscle was significantly correlated with lower back pain(P< 0.01). Conclusion Fractal dimension method can effectively evaluate lumbar paraspinal degeneration, and it is expected to be an objective and quantitative measurement tool for lumbar paraspinal muscle degeneration.
