富血小板血浆联合关节镜治疗中老年半月板损伤的初步临床疗效

Postoperative Effects of Platelet-rich Plasma Combined with Arthroscopy in Treatment of Meniscus Injuries in Elderly Patients

目的:探讨中老年半月板损伤患者应用富血小板血浆联合关节镜治疗的效果及安全性。方法:选取2019年1月-2019年10月我院收治的膝关节半月板损伤患者76例,随机分为对照组和观察组;每组38例,对照组行关节镜手术治疗,观察组手术后于关节腔内注射富血小板血浆。治疗后12个月评定2组治疗效果;分别于治疗前、治疗后3.6、12个月应用视觉模拟评分(Visual analogue scales,VAS)评估膝关节疼痛程度,应用Lysholm膝关节评分评估膝关节功能;记录2组并发症发生情况。结果:治疗后12个月,观察组总有效率为97.36%,高于对照组的81.59%,差异有统计学意义(P<0.05);所有患者治疗后3.6、12个月时VAS评分和Lysholm评分均较治疗前有明显改善,差异均有统计学意义(P<0.05),观察组治疗后VAS评分和Lysholm评分较对照组改善更显著,2组间比较,差异均有统计学意义(P<0.05)。所有患者均无关节内感染、深静脉血栓形成等严重并发症发生,差异均无统计学意义(P>0.05)。结论:中老年半月板损伤患者关节镜术后应用富血小板血浆治疗,可明显减轻疼痛,改善膝关节功能,且对患者的创伤性较小,不增加术后并发症,安全性高,值得临床推广应用。

Objective:To investigate the efficacy and safety of platelet-rich plasma(PRP)combined with arthroscopic surgery in eld-erly patients with meniscus injury.Methods:76 patients with meniscus injury were randomly divided into the control group and the exper-imental group,38 patients in each group.The control group received arthroscopic surgery,and the experimental received platelet-rich plasma in the joint cavity after surgery.The therapeutic effect of the two groups was evaluated 12 months after treatment.V isual analogue scales(VAS)was used to evaluate the pain degree of the knee joint before and 3,6 and 12 months after treatment,respectively.Lysholm knee joint score was used to evaluate the function of the knee joint,and complications in the two groups were recorded.Results:At 12 months after treatment,the total effective rate of the experimental group was 97.36%,higher than that of the control group(81.59%),the difference was statistically significant(P<0.05).VAS scores and Lysholm scores of all patients at 3,6 and 12 months after treatment were significantly improved compared with those before treatment,and the differences were statistically significant(P<0.05).VAS scores and Lysholm scores after treatment were more significantly improved in the experimental group than in the control group,and the differences between the two groups were statistically significant(P<0.05).No serious complications such as intra-articular infection and deep vein thrombosis occurred in all the patients(P>0.05).Conclusion:Platelet-rich plasma therapy after arthroscopy in elderly patients with meniscus injury can significantly reduce pain,improve knee function,and is less invasive for patients,without increasing postoperative complications,high safety,and worthy of clinical application.
