rhBMP-2在后路短节段内固定联合椎体内植骨治疗A型胸腰椎骨折中的临床应用

摘要 目的探讨重组人骨形态发生蛋白-2(rhBMP-2)在后路短节段内固定联合椎体内植骨治疗A型胸腰椎骨折中的应用效果。方法选择2017年3月~2019年3月本科接诊的A型胸腰椎骨折患者100例,按照随机数字法分为对照组(50例)及观察组(50例)。全部患者均采用后路短节段内固定联合椎体内植骨术,对照组采用同种异体碎骨块植入,观察组采用rhBMP-2混合同种异体碎骨块植入。对比两组的手术相关指标及疗效指标。结果两组手术用时、患者失血量、术后引流量、术后下床活动时间、住院时间差异无统计学意义(P>0.05)。两组在术后7 d的Cobb角均较术前显著降低(P<0.05)、术后1年的Cobb角矫正度均有所丢失,但术后1年观察组的Cobb角显著低于对照组(P<0.05)。术后6个月,两组的Lane-Sandhu评分均增高,且观察组显著高于对照组(P<0.05)。观察组的骨折愈合时间显著短于对照组(P<0.05)。两组的术后并发症发生率差异无统计学意义(P>0.05)。结论在A型胸腰椎骨折行后路短节段内固定联合椎体内植骨手术中应用rhBMP-2,有利于促进椎体稳定性恢复,缩短骨折愈合时间。
