持续封闭负压引流术治疗浆细胞性乳腺炎患者的临床疗效

Clinical efficacy of vacuum sealing drainage in the treatment of patients with plasma cell mastitis

目的探讨持续封闭负压引流术(VSD)治疗浆细胞性乳腺炎患者的临床疗效。方法选择青海省中医院2017年2月至2020年2月收治的93例患者,随机分为观察组(n=47)与对照组(n=46)。对照组采用常规切开引流术,观察组采用VSD治疗。比较两组临床疗效、换药频率、创面闭合时间及住院时间、VAS评分及生活质量评分、乳房毁损率及复发率。结果观察组整体临床疗效及临床总有效率均高于对照组,差异有统计学意义(P<0.05)。观察组换药频率、创面闭合时间、住院时间均低于对照组(P<0.001)。治疗后,两组VAS评分下降,生活质量评分升高,差异有统计学意义(P<0.05);且观察组VAS评分低于对照组,生活质量评分高于对照组(P<0.001)。观察组乳房毁损0例,对照组6例(13.04%);疾病复发1例(2.13%),低于对照组的9例(19.57%)(P=0.012,P=0.008)。结论VSD治疗PCM临床疗效显著,可有效改善患者的临床症状体征,减少换药频率、缩短创面闭合时间及住院时间,降低乳房毁损率及疾病复发率,有效缓解患者痛苦,提高其生活质量,值得推广应用。

Objective To explore the clinical efficacy of vacuum sealing drainage(VSD)in treating patients with plasma cell mastitis.Methods A total of 93 patients admitted to Qinghai Hospital of Traditional Chinese Medicine from February 2017 to February 2020 were randomly divided into observation group(n=47)and control group(n=46).The control group was treated with conventional incision and drainage,and the observation group was treated with VSD.The clinical efficacy,frequency of dressing change,wound closure time and length of hospitalization,VAS score and quality of life score,breast damage rate,and recurrence rate were observed and compared between the two groups.Results The overall clinical efficacy and total clinical effectiveness of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).The frequency of dressing change,wound closure time,and length of stay in the observation group was lower than in the control group(P<0.001).After treatment,the VAS score of the two groups decreased,and the quality of life score increased,and the difference was statistically significant(P<0.05).The VAS score of the observation group was lower than that of the control group,and the quality of life score was higher than that of the control group(P<0.001).There were 0 cases of breast damage in the observation group,6 cases in the control group(13.04%).There was 1 case(2.13%)of disease recurrence in the observation group,which was lower than 9 cases(19.57%)in the control group(P=0.012,P=0.008).Conclusion VSD has a significant clinical effect in the treatment of PCM.It can effectively improve the clinical symptoms and signs of patients,reduce the frequency of dressing change,shorten the wound closure time and hospital stay,reduce the breast damage rate and disease recurrence rate,effectively relieve pain,and improve patients'quality of life.It is worthy of being widely popularized.
